IVF-ET/ICSI后发生自然流产的相关因素及预测分析 被引量:1

摘要 目的:探讨不孕症患者行辅助生殖助孕获得宫内妊娠后发生自然流产的相关影响因素。方法:收集2018~2020年在遵义医科大学附属医院生殖医学中心行辅助生殖技术助孕并获宫内妊娠的临床病例750例,分析女方年龄、不孕年限、基础性激素、促排卵方案、受精方式、移植日内膜情况、移植胚胎情况、移植14d血清β-人绒毛膜促性腺激素(β-HCG)情况等,探讨自然流产的相关高危因素。结果:750例临床病例中,112例发生自然流产,638例成功分娩,流产率为14.93%。流产组和分娩组患者的平均年龄、基础FSH值、Gn使用总量及移植后第14d血β-HCG水平比较,差异有统计学意义(P<0.05)。二元logistic回归显示,高龄、高基础FSH值及移植第14d血清β-HCG水平低为新鲜胚胎移植后发生自然流产的独立影响因素。预测自然流产,年龄临界值32.5岁,基础FSH值临界值6.85mIU/mL。移植卵裂期胚胎单胎妊娠与自然流产的第14d血β-HCG水平临界值346.25mIU/mL,移植卵裂期胚胎双胎妊娠临界值408.10mIU/mL。移植囊胚单胎妊娠与自然流产的第14d血β-HCG水平临界值565.10mIU/mL,移植囊胚双胎妊娠临界值904.95mIU/mL。结论:高龄、卵巢功能下降的女性更易发生流产。年龄、基础FSH值及第14d血清β-HCG水平可作为新鲜胚胎移植后自然流产的预测因子。
