渴络欣胶囊联合坎地沙坦酯对早期2型糖尿病肾病患者血清胱抑素C、同型半胱氨酸和炎性因子的影响

The effect of Keluoxin capsule combined with candesartan medoxomil on serum cystatin-C,homocysteine and inflammatory factors in patients with early type 2 diabetic nephropathy

目的 探讨渴络欣胶囊联合坎地沙坦酯对早期2型糖尿病肾病(DN)患者疗效及对血清胱抑素C(CysC)、同型半胱氨酸(Hcy)和炎性因子的影响。方法 选择本院于2019年1月-2021年1月早期DN患者120例,依据随机表法分为治疗组与对照组,各60例。对照组患者口服坎地沙坦酯胶囊,治疗组在坎地沙坦酯胶囊基础上口服渴络欣胶囊。2组疗程12周。比较2组治疗12周总有效率,治疗前与治疗12周肾功能和尿微量白蛋白(mAlb)、CysC、Hcy和白介素、炎性因子和肿瘤坏死因子水平及不良反应情况。结果 治疗组早期DN患者治疗12周总有效率(91.67%)高于对照组(76.67%)(P<0.05),治疗组治疗12周早期DN患者Scr、BUN和mAlb水平低于对照组(P<0.05),治疗组治疗12周早期DN患者血清CysC和Hcy水平低于对照组(P<0.05)。治疗组治疗12周早期DN患者血清IL-6、IL-8和TNF-α水平低于对照组(P<0.05),2组早期DN患者不良反应差异无统计学意义(P> 0.05)。结论 渴络欣胶囊联合坎地沙坦酯对早期DN患者疗效明显,且可降低血清CysC和Hcy水平,减轻微炎症状态。

Objective To investigate the curative effect of Keluoxin capsule combined with candesartan cilexetil on early type 2diabetic nephropathy(DN) and its effect on serum cystatin-C(Cys-C), homocysteine(Hcy) and inflammatory factors.Methods A total of 120 patients with early DN in our hospital from January 2019 to January 2021 were selected and divided into the treatment group and the control group according to a random table method, with 60 cases in each group. Patients in the control group took candesartan cilexetil capsules orally, and the treatment group took Keluoxin capsules on the basis of candesartan cilexetil capsules. The treatment course of 2 groups was 12 weeks. To compare the total effective rate of the two groups of treatment for 12 weeks, the renal function and urine microalbumin(mAlb), Cys-C, Hcy and inflammatory factors[interleukin-6(IL-6), interleukin-8(IL-8) and tumor necrosis factor-α(TNF-α)], and adverse reactions.Results The total effective rate(91. 67%) of early DN patients for 12 weeks was higher in the treatment group than in the control group(76. 67%)(P< 0. 05). The levels of Scr, BUN and mAlb in the treatment group were lower than those in the control group(P< 0. 05). The levels of serum Cys-C and Hcy in the treatment group were lower than those in the control group(P<0. 05). The serum levels of IL-6, IL-8 and TNF-α in the treatment group were lower than those in the control group(P<0. 05). There was no statistical significance on the difference in adverse reactions between the two groups in early DN patients(P> 0. 05).Conclusion Keluoxin capsule combined with candesartan cilexetil has a significant effect on patients with early DN, and it can reduce serum Cys-C and Hcy levels and reduce micro-inflammatory state.
