妊娠早期血清可溶性血管内皮生长因子受体-1、胎盘生长因子水平及两者比值变化与子痫前期发生的相关性

Correlation between serum sFlt- 1, PIGF levels and sFlt - 1/PIGFratio changes in early pregnancy and the occurrence of preeclampsia

目的 观察妊娠早期血清可溶性血管内皮生长因子受体-1(sFlt-1)、胎盘生长因子(PIGF)水平及sFlt-1/PIGF比值变化情况与子痫前期发生的关系。方法 选取2018年2月-2021年6月医院接受常规产前筛查的252例孕妇作为研究对象,于妊娠9周~13周检测孕妇实验室相关指标,于妊娠20周后观察孕妇子痫前期发生情况,分为发生组与未发生组,重点比较2组血清sFlt-1、PIGF水平及sFlt-1/PIGF比值。结果 本研究纳入的252例孕妇中,有14例发生子痫前期,发生率为5.56%;发生组的sFlt-1水平和sFlt-1/PIGF比值均高于未发生组,PIGF水平低于未发生组(P<0.05),组间其他资料对比无差异(P> 0.05);行二元Logistic回归分析后建立多元回归模型,结果显示,sFlt-1过表达、PIGF低表达、sFlt-1/PIGF比值高均是子痫前期发生的影响因素(P<0.05);绘制受试者工作特征曲线(ROC)结果显示,血清sFlt-1、PIGF水平及sFlt-1/PIGF比值预测子痫前期风险的曲线下面积(AUC)分别为0.870、0.958、0.970。结论 子痫前期发生与妊娠早期血清sFlt-1过表达、PIGF低表达及sFlt-1/PIGF高比值有关。

Objective To observe the relationship between the changes of serum soluble vascular endothelial growth factor receptor-1(sFlt-1), placental growth factor(PIGF) levels and sFlt-1/PIGF ratio in early pregnancy and the occurrence of preeclampsia.Methods A total of 252 pregnant women who received routine prenatal screening in the hospital from February2018 to June 2021 were selected as the research objects. Laboratory indicators of pregnant women were detected from 9 weeks to13 weeks of gestation, and the incidence of preeclampsia was observed after 20 weeks of gestation, which was divided into the occurrence group and the non-occurrence group. Serum sF1t-1 and PIGF levels and sF1t-1/PIGF ratio of the two groups were compared.Results Among 252 pregnant women included in this study, 14 had preeclampsia, and the incidence was5. 56%;the sFlt-1 level and sFlt-1/PIGF ratio of the occurrence group were higher than those of the non-occurring group,and the PIGF level was lower than that of the non-occurring group(P< 0. 05). There was no difference of other data between the two groups(P> 0. 05);after performing binary Logistic regression analysis, a multiple regression model was established,and the results showed that overexpression of sFlt-1, low expression of PIGF, and high sFlt-1/PIGF ratio were all influencing factors of preeclampsia(P< 0. 05);the receiver operating characteristic curve(ROC) was drawn, and the results showed that the area under the curve(AUC) of serum sFlt-1, PIGF levels and sFlt-1/PIGF ratio in predicting the risk of preeclampsia were 0. 870, 0. 958, and 0. 970, respectively.Conclusion The occurrence of preeclampsia is related to the overexpression of serum sFlt-1, low expression of PIGF and high ratio of sFlt-1/PIGF in early pregnancy.
