摘要
目的探讨血清淀粉样蛋白A(SAA)、白细胞介素-6(IL-6)、C-反应蛋白(CRP)和降钙素原(PCT)联合检测在新生儿感染性疾病诊断中的应用价值。方法选取滨州市人民医院2021年1月至2022年1月收治的64例感染性疾病新生儿为研究组,75例同期非感染性疾病新生儿为对照组。观察并比较两组间SAA、IL-6、CRP和PCT水平,绘制受试者工作特征曲线(ROC),比较SAA、IL-6、CRP及PCT在新生儿感染性疾病中诊断价值。结果研究组SAA、IL-6、CRP及PCT水平均显著高于对照组,差异均有统计学意义(P均<0.05)。研究组中,血培养阳性新生儿SAA、IL-6、CRP水平均高于血培养阴性新生儿,差异均有统计学意义(P均<0.05),而PCT水平比较差异未见统计学意义(P>0.05)。ROC曲线分析显示,检测SAA和IL-6对新生儿感染性疾病有诊断意义(P均<0.05),CRP和PCT对新生儿感染性疾病无诊断意义(P均>0.05)。联合检测SAA和IL-6对新生儿感染性疾病诊断的敏感度为54.7%,特异度为89.3%。结论SAA、IL-6、CRP及PCT水平在新生儿感染性疾病中升高,联合检测SAA和IL-6对新生儿感染性疾病有诊断意义。
Objective To investigate the value of the combined detection of serum amyloid A(SAA),interleukin-6(IL-6),(C-reactive protein(CRP)and procalcitonin(PCT)in the diagnosis of neonatal infection.Methods From January 2021 to January 2022,64 neonates with infection in Binzhou People’s Hospital(study group)and another 75 neonates with no clinical or biological data of infections were served as the control group during the same period.The serum SAA,IL-6,CRP and PCT levels were observed and compared between the two groups.The value of serum SAA,IL-6,CRP and PCT for the diagnosis of neonatal infection before treatment was assessed by receiver operating characteristic(ROC)curve analysis.Results The serum SAA,IL-6,CRP and PCT levels in the study group were significantly higher than those in the control group,with significant differences(all P<0.05).In study group,the serum levels of SAA,IL-6and CRP in the blood cutures-positive group were significantly higher than those in the blood cutures-negative group,with significant differences(all P<0.05),however,there was no significant differences in the level of PCT(P>0.05).According to the ROC curve,the detection of the serum level of SAA and IL-6 were helpful in the diagnosis of neonatal infection,compared with CRP and PCT.The sensitivity and specificity of combined detection of SAA and IL-6 for neonatal infectious diseases were 54.7%and 89.3%respectively.Conclusions The serum levels of SAA,IL-6,CRP and PCT of neonates with infection are significantly higher than those of neonates with non-infection.The combined detection of serum SAA and IL-6 may be helpful in the diagnosis of neonatal infection.
