布地奈德混悬液雾化吸入联合阿奇霉素序贯治疗对小儿肺炎支原体肺炎的效果观察 被引量:10

Effect of budesonide suspension aerosol inhalation combined with azithromycin sequential therapy on children with Mycoplasma pneumoniae pneumonia

摘要 目的探讨布地奈德混悬液雾化吸入联合阿奇霉素序贯治疗对小儿肺炎支原体肺炎(MPP)的效果。方法抽取联勤保障部队第九八八医院开封医疗区2020年1月至2022年1月收治的78例MPP患儿作为研究对象,按照随机数字表法分为对照组与观察组,每组39例。对照组采用阿奇霉素序贯治疗,观察组采用阿奇霉素序贯治疗联合布地奈德混悬液雾化吸入治疗。比较两组的疗效、症状改善时间、治疗前后肺功能指标[最大呼气流速(PEF)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)]及不良反应发生情况。结果观察组总有效率(94.87%,37/39)高于对照组(76.92%,30/39),P<0.05。观察组体温恢复、咳嗽消失、肺部啰音消失、心率恢复等症状改善时间均短于对照组(P均<0.05)。治疗后,观察组FVC、FEV1、PEF高于对照组(P<0.05)。对照组不良反应发生率(12.82%,5/39)与观察组(7.69%,3/39)比较差异未见统计学意义(P>0.05)。结论布地奈德混悬液雾化吸入联合阿奇霉素序贯治疗小儿MPP可提高疗效,改善肺功能,同时缩短症状改善时间,且不会明显增加不良反应。 Objective To investigate the effect of budesonide suspension aerosol inhalation combined with azithromycin sequential therapy on children with Mycoplasma pneumoniae pneumonia(MPP).Methods Seventy-eight children with MPP admitted to Kaifeng Branch,the 988 Hospital of Joint Logistics Units from January 2020 to January 2022 were selected as the research objects.The patients were divided into control group and observation group by random number table method,with 39 cases in each group.The control group was given azithromycin sequential therapy,and the observation group was given azithromycin sequential therapy combined with budesonide suspension aerosol inhalation therapy.The curative effect,symptom improvement time,pulmonary function indexes including peak expiratory flow(PEF),forced vital capacity(FVC)and forced expiratory volume in 1 second(FEV1),and adverse reactions were compared between the two groups.Results The total effective rate in the observation group(94.87%,37/39)was higher than that in the control group(76.92%,30/39),P<0.05.The improvement time of body temperature recovery,cough disappearance,lung rales disappearance,heart rate recovery in the observation group were shorter than those in the control group(all P<0.05).After treatment,the values of FVC,FEV1 and PEF in the observation group were higher than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the control group(12.85%,5/39)and the observation group(7.69%,3/39),P>0.05.Conclusions Budesonide suspension aerosol inhalation combined with azithromycin sequential therapy in the treatment of children with MPP can improve the curative effect,improve lung function,and shorten the time for symptom improvement without significantly increasing adverse reactions.
