取卵后第1个月经周期进行复苏胚胎移植对妊娠结局和母婴安全的影响:一项回顾性分析

Effect of frozen embryo transfer at the first menstrual cycle after ovum pick-up on pregnancy outcomes and maternal and infant safety:a retrospective study

目的探讨取卵后第1个月经周期进行复苏胚胎移植对妊娠结局及母婴安全的影响。方法回顾性分析2018年1月至2020年9月于郑州大学第二附属医院生殖医学部进行复苏胚胎移植患者的临床资料。根据胚胎冷冻时间和复苏时间,将全胚冻存后第1个月经周期随即进行复苏移植的263例患者设为A组,胚胎复苏时间与新鲜周期冻胚时间间隔≥2个月经周期的2417例患者设为B组。分析两组的临床妊娠率、妊娠丢失率、活产率及母婴安全结局。结果A组的临床妊娠率、妊娠丢失率、活产率、妊娠并发症、胎儿早产率分别为59.3%(156/263)、1.5%(2/133)、48.7%(128/263)、1.1%(3/263)、11.4%(30/263),B组分别为54.1%(1307/2417)、4.6%(52/1129)、44.0%(1063/2417)、0.6%(15/2417)、9.3%(224/2417),两组比较差异未见统计学意义(P>0.05)。结论取卵后第1个月经周期即进行复苏胚胎移植,不影响妊娠结局及母婴安全,且有助于缓解患者的焦虑急迫情绪。

Objective To investigate the effect of frozen embryo transfer at the first menstrual cycle after ovum pick-up on pregnancy outcomes and maternal and infant safety.Methods The clinical data of patients who underwent frozen embryo transfer in Reproductive Center of the Second Affiliated Hospital of Zhengzhou University from January 2018 to September 2020 were analyzed retrospectively.According to the embryo freezing time and transfer time,263 patients who underwent frozen embryo transfer in the first menstrual cycle after whole embryo cryopreservation were set as group A,and 2417 patients whose interval from fresh cycle frozen embryo time to embryo transfer time was≥2 menstrual cycles were set as group B.The clinical pregnancy rate,pregnancy loss rate,live birth rate and maternal and infant safety outcomes of the two groups were analyzed.Results The clinical pregnancy rate,pregnancy loss rate,live birth rate,pregnancy complications and preterm birth rate in the group A were 59.3%(156/263),1.5%(2/133),48.7%(128/263),1.1%(3/263)and 11.4%(30/263),respectively,which had no significant difference with the 54.1%(1307/2417),4.6%(52/1129),44.0%(1063/2417),0.6%(15/2417)and 9.3%(224/2417)in the group B(P>0.05).Conclusions Frozen embryo transfer at the first menstrual cycle after ovum pick-up does not affect pregnancy outcome and maternal and infant safety,and it is conductive to alleviating the anxiety and impatience of patients.
