等速肌力训练对脑卒中后上肢功能障碍患者肢体功能改善的影响 被引量:2

Effect of isokinetic strength training on limb functional recovery of patients with upper limb dysfunction after stroke

摘要 目的分析等速肌力训练对脑卒中后上肢功能障碍患者肢体功能改善的影响。方法抽取2019年3月至2021年3月南阳南石医院收治的脑卒中后上肢功能障碍患者100例,按随机数字表法分为常规组与联合组,每组50例。常规组采用常规康复训练,联合组在常规康复训练基础上联合等速肌力训练。比较两组Fugl-Meyer运动功能测评法(FMA)评分、起立-行走(TUGT)评分、10 m最大步行速度(10mMWS)、Berg平衡量表(BBS)评分、不同角度下屈膝肌峰力矩值、改良Barthel指数(MBI)、日常生活能力。结果干预后,联合组FMA上肢、下肢评分高于常规组(P<0.05)。干预后,联合组TUGT短于常规组,10mMWS高于常规组(P<0.05)。干预前,两组平衡功能评分比较差异未见统计学意义(P>0.05);干预后,联合组平衡功能评分高于常规组(P<0.05)。干预后,联合组屈肌60°/s、120°/s屈膝肌峰力矩值高于常规组(P<0.05)。干预前,两组日常生活能力评分比较差异未见统计学意义(P>0.05);干预后,联合组日常生活能力评分高于常规组(P<0.05)。结论等速肌力训练干预脑卒中后上肢功能障碍,可增加不同角度下屈膝肌峰力矩值,能改善运动、步行和平衡能力,增强日常生活能力。 Objective To analyze the effect of isokinetic strength training on limb functional recovery of patients with upper limb dysfunction after stroke.Methods A total of 100 patients with upper limb dysfunction after stroke treated in Nanshi Hospital of Nanyang from March 2019 to March 2021 were selected.According to the random number table method,all patients were divided into conventional group and combined group,with 50 cases in each group.Routine rehabilitation training was used in the conventional group,and isokinetic strength training was performed in the combined group.Fugl-Meyer assessment scale(FMA)score,time up and go test(TUGT)score,maximum 10 Metre Walk Test(10mMWS)value,Berg balance scale(BBS)score,peak torque of the flexion muscle at different angles,modified Barthel index(MBI),and activities of daily life were compared between the two groups.Results After intervention,the scores of upper and lower limbs of FMA in the combined group were higher than those in the conventional group(P<0.05).After intervention,the TUGT score of the combined group was shorter,and the 10mMWS was higher,compared with those of the conventional group(P<0.05).Before intervention,there was no significant difference in balance function score between the two group(P>0.05);after intervention,the balance function score in the combined group was higher than that in the conventional group(P<0.05).After the intervention,the flexors of 60°/s,120°/s in the combined group were higher than those in the conventional group(P<0.05).Before intervention,there was no significant difference in activities of daily life score(P>0.05);after intervention,the activities of daily life score of the combined group was higher than that of the conventional group(P<0.05).Conclusions Isokinetic strength training can increase the peak moment value of the knee muscles at different angles,improve the ability of locomotion,walking and balance function,and enhance the ability of daily life after stroke.
