穿刺方法对保护动脉内瘘血管的效果研究 被引量:1

摘要 目的研究穿刺方法对保护动脉内瘘血管的效果。方法将自愿参与试验的74例穿刺患者以盲选法平均分组,组别确定为对照组(绳梯式穿刺)、治疗组(扣眼式穿刺)。对两组穿刺质量、血管内膜厚度、穿刺相关并发症发生率进行分析。结果治疗组重复穿刺次数、穿刺紧张度、穿刺疼痛度、压迫止血时间均低于对照组,P<0.05。在穿刺10次、15次之后治疗组内瘘血管内膜壁厚度均低于对照组,P<0.05。治疗组第1个月、3个月、第6个月透析血流均高于对照组,P<0.05。治疗组穿刺相关并发症发生率为2.70%,对照组为16.22%,治疗组发生率低于对照组,P<0.05。结论与绳梯式穿刺相比,扣眼式穿刺降低了穿刺的难度,提高穿刺成功率,对血管的保护作用更加理想,为血液透析提供更好的条件。 Objective To study the effect of puncture on the protection of arterial internal fistula.Methods Seventy-four patients who volunteered to participate in the experiment were randomly divided into two groups:control group(rope ladder puncture)and treatment group(buttonhole puncture).The puncture quality,intima thickness and the incidence of puncture related complications in the two groups were analyzed.Results The number of repeated puncture,puncture tension,puncture pain and compression hemostasis time in the treatment group were lower than those in the control group(P<0.05).After 10 and 15 times of puncture,the intimal wall thickness of fistula vessels in the treatment group was lower than that in the control group(P<0.05).The dialysis blood flow in the treatment group was higher than that in the control group at the 1st,3rd,and 6th months,P<0.05.The incidence of puncture related complications was 2.70%in the treatment group and 16.22%in the control group,which was lower in the experimental group than in the control group(P<0.05).Conclusion Compared with rope ladder puncture,buttonhole puncture reduces the difficulty of puncture,improves the success rate of puncture,and has a more ideal protective effect on blood vessels,providing better conditions for hemodialysis.
