中医优质护理对外固定器治疗三踝骨折术后患者恢复情况的影响

The Effect of High-quality Nursing of Traditional Chinese Medicine on the Recovery of Patients with Trimalleolar Fracture Treated with External Fixator

目的分析三踝骨折特点,探讨中医优质护理干预对外固定架治疗三踝骨折患者恢复情况的影响。方法将2015年11月至2018年11月期间于我院就治行外固定架治疗的三踝骨折患者作为观察对象,总计77例。以随机数字表法分为对照组(n=38)和观察组(n=39),对照组患者接受常规骨科护理干预,观察组患者在对照组护理基础上加用中医优质护理干预。比较两组三踝骨折患者康复情况。以AOFAS踝-后足评分系统(AOFAS Ankle Hindfoot Scale)评估患者的踝关节功能恢复情况,以焦虑自评量表(Self-rating Anxiety Scale,SAS)、抑郁自评量表(Self-rating Depression Scale,SDS)评估患者的焦虑与抑郁情绪状况。结果观察组患者的骨折愈合用时明显短于对照组,P<0.05;观察组针孔感染、骨折愈合延迟等发生率明显低于对照组,P<0.05;术后半年踝关节功能评分与负性情绪评分比较,观察组均明显优于对照组,P<0.05;护理满意度方面,观察组明显高于对照组,P<0.05。结论三踝骨折患者行外固定架治疗后接受中医优质护理干预助于稳定患者的情绪、促进骨折愈合,术后并发症发生减少,踝关节功能恢复好,满足患者护理需求,值得临床应用。

Objective To analyze the characteristics of three-ankle fractures,and to explore the effect of high-quality traditional Chinese medicine nursing intervention on the recovery of patients with trimalleolar fractures treated with external fixators.Methods A total of 77 patients with threeankle fractures who underwent external fixation treatment in our hospital from November 2015 to November 2018 were taken as observation objects.The patients were divided into the control group(n=38)and the observation group(n=39)according to the random number table.The patients in the control group received conventional orthopedic nursing intervention,and the patients in the observation group received high-quality traditional Chinese medicine nursing intervention on the basis of the control group nursing.The rehabilitation of patients with trimalleolar fractures between the two groups was compared.AOFAS Ankle Hindfoot Scale(AOFAS Ankle Hindfoot Scale)was used to assess the patient’s ankle function recovery,and the Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS)assess the patient’s anxiety and depression emotional state.Results The fracture healing time of the observation group was significantly shorter than that of the control group,P<0.05;the incidence of pinhole infection and delayed fracture healing in the observation group was significantly lower than that of the control group,P<0.05;the ankle joint function score and negative sexual emotion scores of the observation group were significantly better than the control group for a half a year after the operation,P<0.05;in terms of nursing satisfaction,the observation group was significantly higher than the control group,P<0.05.Conclusion Patients with trimalleolar fractures undergoing external fixation treatment and receiving high-quality TCM nursing interventions can help stabilize the patient’s mood,promote fracture healing,reduce postoperative complications,restore ankle joint function,and meet the patient’s nursing needs.It is worthy of clinical application.
