四川省大骨节病消除达标后巩固评价结果分析

Analysis of consolidation evaluation results after elimination of Kashin-Beck disease in Sichuan Province

目的 评价四川省大骨节病病情及防控措施效果。方法 按照《重点地方病控制和消除评价办法(2019版)》,在32个病区县开展大骨节病消除达标后巩固评价县级自评和省级抽查。县级自评包括自首次达到消除标准以来大骨节病监测7~12周岁儿童病情及防控措施落实情况,省级在每个市(州)随机抽取1~2个县复核儿童病情和防控措施落实情况,并进行结果判定。结果 县级自评结果:自首次达到消除大骨节病标准以来(2013—2019年),32县所有病区村均未检出7~12周岁儿童大骨节病临床病例;X线阳性检出率为0~0.19%,其中2019年为0,X线阳性改变均为干骺端改变;各病区村不同程度落实了退耕还林(草)、改种经济作物、换粮、搬迁、易地育人、补硒等综合防治措施;居民主要以大米为主食。省级抽查结果:抽查了9个县,对2 503名7~12岁儿童进行大骨节病临床检查,均未检出临床病例;随机抽查的443户居民家庭防控措施落实情况与县级自评基本一致。结论 四川省32个大骨节病病区县均保持消除大骨节病状态。

Objective To evaluate the condition of Kashin-Beck disease(KBD) in Sichuan Province and the effect of prevention and control measures.Methods In accordance with the "Evaluation methods for the control and elimination of key endemic diseases(2019 Edition)",the county-level self-assessment and provincial-level spot check of consolidation evaluation after elimination of KBD were carried out in 32 endemic counties.The county-level self-assessment included the monitoring of KBD in children aged 7-12 years and the implementation of prevention and control measures since the elimination standard was first reached.At the provincial level, 1-2 counties were randomly selected from each city to review the situation of children’s disease and the implementation of prevention and control measures and determine the results.Results County-level self-assessment results: since the KBD elimination standard were first reached(2013—2019),no clinical cases of children aged 7-12 years were detected in all endemic villages in 32 counties;The positive X-ray detection rate was 0~0.19%,among them, it was 0 in 2019,and the X-ray positive changes were all metaphysis changes;Comprehensive prevention and control measures, such as returning farmland to forest(grass),planting cash crops, exchanging grain, relocating, educating people and supplementing selenium in non-endemic places, were implemented to varying degrees in endemic villages.The residents mainly live on rice.Provincial-level spot check results: 9 counties were randomly checked, 2 503 children aged 7-12 were clinically examined for KBD,and no clinical cases were found.The implementation of prevention and control measures in 443 households randomly checked was basically consistent with the county-level self-assessment.Conclusions All 32 endemic counties in Sichuan Province have maintained the status of KBD elimination.
