经尿道前列腺等离子电切术治疗前列腺增生合并前列腺炎疗效分析

Analysis of the curative effect of transurethral resection of the prostate in the treatment of benign prostatic hyperplasia complicated with prostatitis

目的 分析经尿道前列腺等离子电切术治疗前列腺增生合并前列腺炎的疗效.方法 选取2020年5月-2021年5月期间在湖北省鄂州市鄂钢医院治疗的前列腺增生合并前列腺炎患者50例为研究对象,所有患者均采用经尿道前列腺等离子电切术.观察手术前、后3个月患者的手术指标、最大尿流率(Q_(max))、中文版国际前列腺症状评分(IPSS)、美国国立卫生研究院慢性前列腺炎症状指数(NIH-CPSI)、性功能.结果 所有患者的手术时间、术中出血量、尿管留置时间、造瘘管留置时间、住院时间分别为(50.68±8.52)min、(98.65±19.64)ml、(4.24±0.46)d、(5.38±1.27)d、(7.65±1.16)d.患者术后Q_(max)(17.21±3.87)ml/s高于术前(8.35±1.51)ml/s,IPSS评分(8.29±2.14)分、NIH-CPSI评分(10.24±2.72)分均低于术前的(21.68±4.26)分、(18.95±3.15)分,差异有统计学意义(P<0.05);手术前后的性功能状况比较,无明显差异(P>0.05).结论 前列腺增生合并前列腺炎患者行经尿道前列腺等离子电切术,疗效显著,且不会影响性功能.

Objective To analyze the efficacy of transurethral resection of the prostate(TURP)in the treatment of benign prostatic hyperplasia complicated with prostatitis.Methods A total of 50 patients with benign prostatic hyperplasia and prostatitis who were treated in our hospital from May 2020 to May 2021 were selected as the research objects,and all patients underwent transurethral resection of the prostate.The surgical indexes,maximum urinary flow rate(Q_(max)),Chinese version of International Prostate Symptom Score(IPSS),National Institutes of Health Chronic Prostatitis Symptom Index(NIH-CPSI),and sexual function were observed before and 3 months after surgery.Results The operation time,intraoperative blood loss,urinary catheter indwelling time,fistula indwelling time and hospitalization time of all patients were(50.68±8.52)min,(98.65±19.64)ml,(4.24±0.46)d,(5.38±1.27)d,(7.65±1.16)d respectively.Postoperative Q_(max)(17.21±3.87)ml/s was higher than preoperative(8.35±1.51)ml/s,IPSS score(8.29±2.14)and NIH-CPSI score(10.24±2.72)were lower than preoperative(21.68±4.26)points,(18.95±3.15)points,the difference was statistically significant(P<0.05);there was no significant difference in sexual function status before and after surgery(P>0.05).Conelusion Transurethral plasma resection of the prostate in patients with benign prostatic hyperplasia and prostatitis has a significant effect without affecting sexual function.
