PDCA循环在降低中心静脉导管非计划性拔管率中的应用

Application of PDCA circulation in reducing unplanned extubation rate of central venous catheter

目的探讨PDCA循环在降低中心静脉导管非计划性拔管率中的应用效果.方法应用PDCA循环的四个阶段八个步骤,对福建省级机关医院2020年中心静脉导管非计划性拔管原因分析,制定改进计划并组织实施,观察并比较PDCA循环前后患者约束效果及中心静脉导管非计划性拔管发生率.结果通过应用PDCA循环,护士对中心静脉导管识别率及重视度由运用前的79.7%上升至92.9%;患者约束效果由应用前的75.6%上升至86.7%;中心静脉导管非计划性拔管发生率由应用前的0.88‰降至0.19‰,差异有统计学意义(P<0.05).结论运用PDCA循环提高了护士预防非计划性拔管的识别率,优化患者约束方式,提高约束效果,降低了中心静脉导管非计划性拔管的发生率,值得临床借鉴.

0bjective To investigate the effect of PDCA circulation in reducing the rate of unplanned extubation of central venous catheter.Methods Four stages and eight steps of PDCA cycle were used to analyze the causes of unplanned extubation of central venous catheter in our hospital in 2020,formulate improvement plans and organize implementation,observe and compare the restraint effect of patients before and after PDCA cycle and the incidence of unplanned extubation of central venous.catheter.Results After PDCA circulation,the recognition rate and importance of central venous catheter increased from 79.7%to 92.9%.The patient restraint effect increased from 75.6%before application to 86.7%;the incidence of unplanned catheter in central venous catheter decreased from 0.88%o to 0.19%o,the difference was statistically significant(P<0.05).Conclusion PDCA cycle can improve the identification rate of nurses to prevent unplanned extubation,optimize the restraint mode of patients,improve the restraint effect,and reduce the incidence of unplanned extubation of central venous catheter,which is worthy of clinical reference.
