早期康复联合高压氧治疗缺血性脑卒中患者功能愈后的临床效果

Clinical effect of early rehabilitation combined with hyperbaric oxygen therapy on the functional recovery of patients with ischemic stroke

目的探讨早期康复联合高压氧治疗缺血性脑卒中患者功能愈后的临床疗效。方法64例缺血性脑卒中患者随机均分为两组:对照组在常规药物治疗基础上进行早期康复治疗;而研究组在对照组基础上同时联合高压氧治疗。于治疗前及治疗2、4周后,采用Fugl-Meyer量表、改良Barthel指数量表(MBI)、简易精神状态检查量表(MMSE)、美国国立卫生研究院卒中量表(NIHSS)和SF-36量表分别评估患者肢体运动功能、日常生活活动能力、认知功能、神经功能和生活质量。结果与治疗前相比,两组治疗后Fugl-Meyer、MBI、MMSE及SF-36评分增加,NIHSS评分降低(P<0.05);研究组治疗4周后Fugl-Meyer、MBI及SF-36评分高于治疗2周后,NIHSS评分低于治疗2周后(P<0.05)。研究组治疗4周后Fugl-Meyer、MBI、MMSE及SF-36评分高于对照组,NIHSS评分低于对照组(P<0.05)。结论早期康复联合高压氧治疗可改善缺血性脑卒中患者肢体运动功能、认知功能和神经功能缺损程度,提高日常生活活动能力和生活质量。

Objective To investigate the clinical efficacy of early rehabilitation combined with hyperbaric oxygen therapy in the treatment of functional recovery of the patients with ischemic stroke.Methods Sixty-four patients with ischemic stroke were equally randomized into two groups.The patients in group A accepted early rehabilitation on the basis of regular medication,who in group B received additional hyperbaric oxygen therapy.The motor function,activity of daily living,cognitive function,neurological function and life quality before treatment and after 2 and 4 weeks of treatment were measured by Fugl-Meyer scale,modified Barthel index(MBI),mini-mental state examination(MMSE),National Institutes of Health stroke scale(NIHSS)and SF-36 scale,respectively.ResultsCompared with before,the Fugl-Meyer,MBI,MMSE and SF-36 scores were increased,while NIHSS score was decreased in groups of A and B after treatment(P<0.05).The Fugl-Meyer,MBI and SF-36 scores after 4 weeks of treatment were higher and NIHSS score was lower than those after 2 weeks of treatment in group B(P<0.05).Compared with group A,the Fugl-Meyer,MBI,MMSE and SF-36 scores were increased,while NIHSS score was decreased in group B after 4 weeks of treatment(P<0.05).Conclusion The early rehabilitation combined with hyperbaric oxygen therapy could effectively improve the motor function,cognitive function and neurological function,and enhance the activity of daily living and life quality in the patients with ischemic stroke.
