Treatment of damp-heat syndrome of IgA nephropathy based on the differentiation of sanjiao

IgA肾病是临床范围内常见的原发性肾小球疾病。现代医家多从湿热、虚、血瘀等方面论治本病。其中湿热证在IgA肾病发病机制中起着重要作用,是重要的邪实之一,它贯穿于疾病的整个过程,是影响疾病发展和转归的重要因素。而湿热证的病机为肺脾肾气虚,易感受湿热外邪或肺、脾、肾三脏之输布运化蒸腾失司而使湿热内生。本文通过对IgA肾病病因病机,以及湿热证的病机,从肺肾同治、脾肾同治、肺脾肾同治三个角度论治IgA肾病湿热证。

IgA nephropathy is a common primary glomerular disease in clinic.Modern physicians mostly treat this disease from the aspects of damp-heat,deficiency and blood stasis.Among them,the damp-heat syndrome plays an important role as the pathogenesis of IgA nephropathy and one of the important excessive pathogens.The syndrome runs through the whole process of IgA nephropathy and is an important factor affecting the development and outcome of the disease.While the pathogenesis of the syndrome is either the deficiency of lung,spleen and kidney qi,rendering the body subject to damp-heat exogenous pathogens or the failure of the three organs in their functions of transportation,transformation and transpiration,resulting in the damp-heat endogenesis.Based on the etiology and pathogenesis of IgA nephropathy,as well as the pathogenesis of the damp-heat syndrome,this paper discusses the treatment of the damp-heat syndrome in IgA nephropathy from three perspectives:the simultaneous treatment of the lung and kidney,the simultaneous treatment of the spleen and kidney,and the simultaneous treatment of the lung,spleen and kidney.
