基于5G的远程机器人辅助胸腰椎骨折内固定术的临床应用研究 被引量:2

摘要 目的:分析远程机器人辅助骨折复位内固定术治疗胸腰椎骨折的临床效果,为基于5G的远程机器人辅助骨科手术的应用与推广提供依据。方法:回顾性分析2021年4月—12月由北京积水潭医院脊柱外科医师远程操作外院手术机器人完成的骨折复位内固定术48例患者临床信息。统计患者骨折固定效果、并发症和远程通信成功率,评估远程机器人手术的临床应用效果。结果:48例远程机器人辅助胸腰椎骨折内固定术均顺利完成,平均手术时间为(93.23±33.37)min。共置入了288枚椎弓根螺钉,254枚(88.19%)螺钉为A级,26枚(9.03%)螺钉为B级,8枚(2.78%)螺钉为C级。螺钉位置的临床可接受率(A级+B级)为97.22%。术后的骨折椎体前缘高度比显著大于术前(P<0.001)。远程通信成功率为100%,未出现围手术期并发症。结论:远程机器人辅助胸腰椎骨折内固定术可获得良好的骨折固定效果,基于5G的远程机器人辅助胸腰椎骨折内固定术是安全、有效的。 Objective To analyze the clinical effect of remote robot-assisted fracture reduction and internal fixation in the treatment of thoracolumbar fracture,and provide basis for the application and promotion of remote robot-assisted orthopedic surgery based on 5G.Methods The clinical information of 48 patients with fracture reduction and internal fixation completed by spine surgeons of Beijing Jishuitan Hospital remotely operating the surgical robot of another hospital from April to December 2021 were analyzed retrospectively.The effect of fracture fixation,complications and the success rate of remote communication were analyzed,and the clinical application effect of remote robot surgery was evaluated.Results 48 cases of remote robot-assisted internal fixation surgeries for thoracolumbar fracture were successfully completed,and the average operation time was(93.23±33.37)min.A total of 288 pedicle screws were implanted,of which 254(88.19%)screws were Grade A,26(9.03%)screws were Grade B and 8(2.78%)screws were Grade C.The clinical acceptability rate of the screw position(Grade A+Grade B)was 97.22%.The height ratio of anterior edge of vertebral body in postoperative fractures was higher than that before surgery(P<0.001).The success rate of remote communication was 100%,and there were no perioperative complication.Conclusion Remote robotassisted thoracolumbar fracture internal fixation can achieve good fracture fixation effect,and remote robot-assisted thoracolumbar fracture internal fixation based on 5G network is safe and effective.
