血清Hs-cTnT、BNP水平与慢性心力衰竭患者NYHA心功能分级的相关性及临床意义 被引量:7

Correlations of serum Hs-cTnT and BNP levels with NYHA cardiac function classification in patients with chronic heart failure and their clinical significance

摘要 目的:观察血清高敏心肌肌钙蛋白T(Hs-cTnT)、B型脑钠肽(BNP)水平与慢性心力衰竭(CHF)患者美国纽约心脏病学会(NYHA)心功能分级的相关性及临床意义。方法:选取2019年11月至2020年11月本院收治的102例CHF患者作为观察组,选取同期100名健康体检者为对照组,进行病例对照研究,比较两组血清Hs-cTnT、BNP水平,并比较不同NYHA心功能分级CHF患者血清Hs-cTnT、BNP水平,采用Spearman相关性分析法分析血清Hs-cTnT、BNP水平与NYHA心功能分级相关性。随访6个月,统计CHF患者主要心脏不良事件(MACE)发生情况,比较不同MACE发生情况CHF患者血清Hs-cTnT、BNP水平。结果:观察组血清Hs-cTnT、BNP水平高于对照组,差异有统计学意义(P<0.05);NYHA心功能分级Ⅳ级CHF患者血清Hs-cTnT、BNP水平高于Ⅱ、Ⅲ级,且Ⅲ级CHF患者血清Hs-cTnT、BNP水平高于Ⅱ级,差异有统计学意义(P<0.05);Spearman相关性分析结果显示,血清Hs-cTnT、BNP水平与NYHA心功能分级呈正相关(P<0.05);随访6个月,发生MACE的CHF患者血清Hs-cTnT、BNP水平高于未发生MACE的CHF患者,差异有统计学意义(P<0.05)。结论:CHF患者血清Hs-cTnT、BNP水平与NYHA心功能分级呈正相关,检测血清Hs-cTnT、BNP水平可为临床治疗及预后评估提供参考依据。 Objective: To observe correlations of serum high-sensitivity cardiac troponin T(Hs-cTnT) and B-type brain natriuretic peptide(BNP) levels with New York heart association(NYHA) cardiac function classification in patients with chronic heart failure(CHF) and their clinical significance. Methods: 102 CHF patients admitted to our hospital from November 2019 to November 2020 were selected as observation group, and 100 healthy subjects during the same period were selected as control group. They were included in a case-control study. The levels of serum Hs-cTnT and BNP were compared between the two groups. The serum Hs-cTnT and BNP levels in the CHF patients with different NYHA cardiac function classification s were compared. Spearman correlation analysis was used to analyze the correlations among the serum Hs-cTnT and BNP levels and the NYHA cardiac function classification. After 6 months of follow-up, the incidence of major adverse cardiac events(MACE) in the CHF patients was statistically analyzed. Then, the serum Hs-cTnT and BNP levels of the CHF patients with different MACE occurrence conditions were compared.Results: The serum Hs-cTnT and BNP levels in the observation group were higher than those in the control group, and the differences were statistically significant(P<0.05). The levels of serum Hs-cTnT and BNP in the CHF patients with NYHA grade IV were higher than those with grades II and III;the levels of serum Hs-cTnT and BNP in the CHF patients with NYHA grade III were higher than those with grade II;and the differences were statistically significant(P<0.05). The Spearman correlation analysis showed that serum Hs-cTnT and BNP levels were positively correlated with the NYHA cardiac function classification(P<0.05). During the 6-month follow-up, the serum Hs-cTnT and BNP levels in the patients with MACE CHF were higher than those in the patients without MACE CHF, and the differences were statistically significant(P<0.05). Conclusions: The serum Hs-cTnT and BNP levels in the CHF patients were positively correlated with the NYHA cardiac function classification. Therefore, the detection of serum Hs-cTnT and BNP levels can provide reference for the clinical treatment and prognosis evaluation.
