彩色多普勒超声在乳腺肿块诊断中的应用效果观察

Application effect of color Doppler ultrasonography in the diagnosis of breast masses

目的 分析彩色多普勒超声诊断乳腺肿块的效果,探讨其临床应用价值。方法 150例乳腺肿块患者作为研究对象,均进行常规二维超声检查及彩色多普勒超声检查。以最终病理诊断结果为金标准,比较二维超声和彩色多普勒超声诊断乳腺肿块的灵敏度、特异度和准确度;观察患者的超声诊断影像学特征,统计彩色多普勒超声诊断下患者的血流信号分级情况。结果 经最终病理诊断,150例乳腺肿块患者中,良性乳腺肿块患者97例(64.67%),恶性乳腺肿块患者53例(35.33%)。二维超声和彩色多普勒超声诊断乳腺肿块的灵敏度分别为66.04%和92.45%,特异度分别为90.72%和97.94%,准确度分别为82.00%和96.00%。彩色多普勒超声诊断乳腺肿块的灵敏度、特异度、准确度均显著高于二维超声,差异具有统计学意义(P<0.05)。彩色多普勒超声检查显示,恶性乳腺肿块患者的血流信号差于良性乳腺肿块,差异具有统计学意义(Z=8.459,P<0.05)。结论 彩色多普勒超声应用于乳腺肿块的诊断中,既能提供肿块的二维影像学信息,又能提供病灶的血流信号变化情况,为肿块良恶性的鉴别提供依据,诊断准确度高,值得临床推广应用。

Objective To analyze the effect of color Doppler ultrasonography in the diagnosis of breast masses,and discuss its clinical application value.Methods 150 patients with breast masses were studied,and both conventional two-dimensional ultrasonography and color Doppler ultrasonography were performed.The sensitivity,specificity and accuracy of two-dimensional ultrasound and color Doppler ultrasound in diagnosing breast masses were compared using the final pathological diagnosis as the gold standard.The ultrasound diagnostic imaging features of the patients were observed,and the blood flow signal grading of the patients under color Doppler ultrasonography diagnosis was counted.Results According to final pathological diagnosis,97 patients(64.67%)had benign breast masses and 53 patients(35.33%)had malignant breast masses were among the 150 patients with breast masses.The sensitivity of two-dimensional ultrasonography and color Doppler ultrasonography in the diagnosis of breast masses were 66.04%and 92.45%,the specificity was 90.72%and 97.94%,and the accuracy were 82.00%and 96.00%.The sensitivity,specificity and accuracy of color Doppler ultrasonography in diagnosing breast masses were significantly higher than those of two-dimensional ultrasonography,and the differences were statistically significant(P<0.05).Color Doppler ultrasonography showed that the blood flow signal of malignant breast masses was worse than that of benign breast masses,and the difference was statistically significant(Z=8.459,P<0.05).Conclusion The application of color Doppler ultrasonography in the diagnosis of breast masses can not only provide the two-dimensional imaging information of the masses,but also provide the changes of the blood flow signals of the lesions,which can provide a basis for the identification of benign and malignant masses with high diagnostic accuracy.It is worthy of clinical promotion and application.
