胎儿心律失常的超声检测方法

摘要 目的探讨快速、有效的胎儿心律失常的超声检测方法。方法选取2019年10月—2020年9月在上海市嘉定区妇幼保健院就诊的500例孕妇为研究对象,检测心脏M型超声联合左室流入及流出道,或肺静脉和外周肺动脉,或升主动脉和上腔静脉交汇处脉冲多普勒,比较有效率。结果M型超声、左室流入及流出道多普勒、升主动脉和上腔静脉多普勒及肺静脉和外周肺动脉多普勒检测成功率分别为89.2%、81.0%、56.4%及92.8%,差异有统计学意义(χ^(2)=129.87,P<0.05)。左室流入及流出道多普勒,升主动脉和上腔静脉多普勒及肺静脉和外周肺动脉普勒检测成功率两两比较差异均有统计学意义(χ^(2)=70.36,30.58,174.81,均P<0.05)。结论通过M型超声联合肺静脉和外周肺动脉多普勒检测胎儿心律失常准确、快速、有效,在需要判断房室传导时间时,可采用左室流入及流出道频谱测量,为临床诊断提供准确、可靠的依据。 Objective To explore rapid and effective ultrsonic detection methods of fetal arrhythmia.Methods From October 2019 to September 2020,500 pregnant women were selected from Maternal and Child Health Care Hospital of Jiading District,Shanghai as study object,cardiac M-type ultrasound combined with left ventricular inflow and outflow tract,or pulmonary vein and peripheral pulmonary artery,or ascening aorta and superior vena cava junction pulse Doppler were detected,the effective rates were compared.Results The successful rates of cardiac M-type ultrasound,left ventricular inflow and outflow tract,ascening aorta and superior vena cava,and pulmonary vein and peripheral pulmonary artery pulse Doppler were 89.2%,81.0%,56.4%,and 92.8%,respectively,there was statistically significant difference(χ^(2)=129.87,P<0.05).There were statistically significant differences in the successful rate among any two means of left ventricular inflow and outflow tract,ascening aorta and superior vena cava,and pulmonary vein and peripheral pulmonary artery(χ^(2)=70.36,30.58,174.81,all P<0.05).Conclusion Cardiac M-type ultrasound combined with pulmonary vein and peripheral pulmonary artery pulse Doppler in detection of fetal arrhythmia is accurate,rapid,and effective,left ventricular inflow and outflow tract can be used for spectrum measurement when juding atrioventricular conduction time to provide an accurate and reliable basis for clinical diagnosis.
