关怀理念为导向的照护策略对急性髓系白血病患者心理状态及癌因性疲乏程度的影响

The Effect of Caring Concept-oriented Nursing Strategy on the Psychological State and Cancer-related Fatigue of Patients with Acute Myeloid Leukemia

目的:探究以关怀理念为导向的照护策略对急性髓系白血病(AML)患者的心理状态及癌因性疲乏程度的影响。方法:选取2018年4月—2020年6月郑州市第三人民医院收治的83例AML患者作为研究对象,根据建档时间不同分为两组,其中41例行常规护理为对照组,另42例在常规护理基础上结合关怀理念为导向的照护策略为观察组,比较两组患者干预前后的不良心理状态[焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分]、癌因性疲乏(PFS)、睡眠障碍(RCSQ)、生活质量(QLQ-C30)。结果:干预后观察组SAS、SDS评分较对照组低,差异有统计学意义(t=8.127、5.280,P<0.05)。干预后观察组PFS量表躯体疲乏、认知疲乏、情感疲乏、综合疲乏维度评分均低于对照组,差异有统计学意义(t=10.681、3.919、8.704、5.604,P<0.05)。干预后观察组RCSQ量表睡眠质量、重回睡眠、睡眠期潜伏、夜间觉醒、睡眠深度维度评分均高于对照组,差异有统计学意义(t=6.196、7.400、4.735、5.028、5.765,P<0.05)。干预后观察组QLQ-C30评分高于对照组,差异有统计学意义(t=6.138,P<0.05)。结论:关怀理念为导向的照护策略应用于AML患者,可调节心理状态,降低癌因性疲乏程度,改善睡眠,提高生活质量。

Objective:To analyze the effect of caring concept-oriented nursing strategies on the psychological state and cancer-related fatigue of patients with acute myeloid leukemia(AML).Methods:A total of 83 AML patients admitted to the hospital from April 2018 to June 2020 were selected as the research subjects,and divided into two groups according to the time of file establishment.Among them,41 routine nursing cases were selected as the control group,and the other 42 cases were selected as the observation group based on the routine nursing combined with the care concept-oriented nursing strategy.The adverse psychological state(self-rating anxiety scale[SAS]score,self-rating depression scale[SDS]score),cancer-related fatigue(PFS),sleep disturbance(RCSQ),quality of life(QLQ-C30)were compared between the two groups before and after the intervention.Results:After intervention,the SAS and SDS scores of the observation group were lower than those of the control group,and the difference was statistically significant(t=8.127,5.280,P<0.05).After the intervention,the physical fatigue,cognitive fatigue,emotional fatigue,and comprehensive fatigue dimension scores of the observation group were lower than those of the control group,and the differences were statistically significant(t=10.681,3.919,8.704,5.604,P<0.05).After the intervention,the RCSQ scale sleep quality,return to sleep,sleep phase latency,nocturnal awakening,and sleep depth dimension scores in the observation group were higher than those in the control group,and the differences were statistically significant(t=6.196,7.400,4.735,5.028,5.765,P<0.05).After the intervention,the QLQ-C30 score of the observation group was higher than that of the control group,and the difference was statistically significant(t=6.138,P<0.05).Conclusion:The nursing concept-oriented nursing strategy applied to AML patients can adjust the psychological state,reduce the degree of cancer-related fatigue,improve sleep,and improve the quality of life.
