Influencing Factors and Nursing Countermeasures of Adverse Reactions Related to Iodine Contrast Agent in En⁃hanced CT Scan

摘要 目的:探究增强CT扫描中碘对比剂相关不良反应影响因素及护理对策。方法:回顾性分析2018年8月—2019年8月在焦作市人民医院行增强CT扫描的604例患者的临床资料,观察患者注射碘对比剂后不良反应发生情况,分析增强CT扫描中碘对比剂发生不良反应的危险因素。结果:604例患者中,增强CT扫描发生碘对比剂相关不良反应11例,男7例,女4例,轻度8例,中度2例,重度1例,不良反应发生率为1.82%(11/604);logistic多因素回归分析表明,年龄<50岁、注射速率≥2.5 mL/s、注射剂量≥1.4 mL/kg是患者碘对比剂不良反应的危险因素。结论:年龄、注射速率及注射剂量是增强CT扫描中碘对比剂相关不良反应影响因素,在保证CT增强效果的前提下,应尽量减慢对比剂的注射速率,减少对比剂注射剂量。 Objective:To explore the influencing factors and nursing strategies of iodine contrast agent-related adverse reac⁃tions in enhanced CT scans.Methods:The clinical data of 604 patients who underwent enhanced CT scans in the hospital from August 2018 to August 2019 were retrospectively analyzed.The incidence of adverse reactions after injection of iodine contrast agent in patients was observed,and the risk factors for adverse reactions of iodine contrast agent in contrast-enhanced CT scan were analyzed.Results:Among the 604 patients,11 cases of iodine contrast agent-related adverse reactions occurred on enhanced CT scan,including 7 males and 4 females,8 mild,2 moderate,and 1 severe,with an adverse reaction rate of 1.82%(11/604).Lo⁃gistic multivariate regression analysis showed that age<50 years old,injection rate≥2.5 mL/s,and injection dose≥1.4 mL/kg were risk factors for adverse reactions of iodine contrast agents.Conclusion:Age,injection rate and injection dose are influencing factors of iodine contrast-related adverse reactions in contrast-enhanced CT scans.On the premise of ensuring the CT enhance⁃ment effect,the injection rate of the contrast agent should be slowed down as much as possible,and the injection dose of the con⁃trast agent should be reduced.
