造血干细胞捐献后电解质和骨代谢相关指标变化及意义

Changes and significance of electrolytes and bone metabolism-related indexes after hematopoietic stem cell donation

摘要 目的探讨干细胞捐献者柠檬酸钠抗凝剂对电解质和骨代谢相关指标的影响, 评估其对干细胞捐献者骨密度(BMD)的影响, 更好地保护干细胞捐献者的健康。方法采集23名正常外周血干细胞捐献者注射动员剂人类粒细胞集落刺激因子(G-CSF)前24 h及干细胞采集后6个月的血样, 分别进行电解质以及骨代谢相关指标检测, 包括游离钙离子(Ca2+)、镁离子(Mg2+)、钾离子(K+)、钠离子(Na+)、氯离子(Cl-)、无机磷(Pi);骨形成指标骨钙素(OC)、β-胶原特殊序列(β-CTX)、碱性磷酸酶(ALP)、BMD、总Ⅰ型胶原氨基端延长肽(T-PINP)和25羟维生素D(25-OHVD)。结果注射动员剂G-CSF前24 h及干细胞采集后6个月血清中Ca2+(2.35±0.06比2.41±0.06)、Mg2+(0.89±0.05比0.89±0.06)、K+(4.29±0.30比4.18±0.34)、Na+(142.26±1.83比141.21±2.31)、Cl-(2.35±0.06比2.41±0.06)水平差异无统计学意义(P>0.05), Pi水平升高(0.47±0.42比0.63±0.39, P<0.05);注射动员剂G-CSF前24 h及干细胞采集后6个月OC(14.46±3.79比14.71±3.03)、β-CTX(0.44±0.18比0.41±0.18)、ALP(69.82±19.43比80.87±23.87)、BMD(0.95±0.09比0.91±0.15)、T-PINP(48.59±19.00比46.02±16.54)差异无统计学意义(P>0.05), 25-OHVD水平明显降低(21.09±7.13比19.94±6.34, P<0.05)。结论注射动员剂和柠檬酸钠抗凝剂对造血干细胞捐献后电解质和骨代谢相关指标无明显影响, 干细胞捐献者采集前后未出现明显不良反应。 Objective To investigate the effect of sodium citrate anticoagulant on electrolytes and bone metabolism-related indexes in stem cell donors,so as to evaluate its effect on bone mineral density(BMD)of stem cell donors and better protect the health of stem cell donors.Methods Blood samples were collected from 23 normal peripheral blood stem cell donors in 24 h before injection of mobilizing agent granulocyte colony-stimulating factor(G-CSF)and 6 months after stem cell collection.Electrolytes and bone metabolism-related indexes were detected respectively,including free calcium ion(Ca2+),magnesium ion(Mg2+),potassium ion(K+),sodium ion(Na+),chloride ion(Cl-),inorganic phosphorus(Pi);bone formation indicator osteocalcin(OC),β-collagen specific sequence(β-CTX),alkaline phosphatase(ALP),BMD,total typeⅠcollagen N-terminal extension peptide(total typeⅠpro-collagen N-terminal perminal peptide,T-P1NP)and 25-hydroxyvitamin D(25-OHVD).Results There was no significant difference in serum Ca2+(2.35±0.06 vs.2.41±0.06),Mg2+(0.89±0.05 vs.0.89±0.06),K+(4.29±0.30 vs.4.18±0.34),Na+(142.26±1.83 vs.141.21±2.31),Cl-(2.35±0.06 vs.2.41±0.06)levels in 24 h before injection of mobilizing agent G-CSF and 6 months after stem cell collection(P>0.05),but Pi level increased(0.47±0.42 vs.0.63±0.39,P<0.05).There was no significant difference in OC(14.46±3.79 vs.14.71±3.03),β-CTX(0.44±0.18 vs.0.41±0.1),ALP(69.82±19.43 vs.80.87±23.87),BMD(0.95±0.09 vs.0.91±0.15),T-PINP(48.59±19.00 vs.46.02±16.54)in 24 h before mobilization of G-CSF and 6 months after stem cell collection(P>0.05),but the level of 25-OHVD was significantly decreased(21.09±7.13 vs.19.94±6.34,P<0.05).Conclusion The injection of mobilizing agent and sodium citrate anticoagulant has no significant effect on electrolytes and bone metabolism-related indexes after hematopoietic stem cell donation,and there is no obvious adverse reaction in stem cell donors before and after collection.
