2型糖尿病患者胰岛素抗体水平与尿白蛋白/肌酐比值的相关性研究 被引量:1

Correlation between insulin antibody and urinary albumin/creatinine ratio in patients with type 2 diabetes mellitus

摘要 目的 探讨T2DM患者胰岛素抗体(Ins-Ab)水平与UACR的相关性。方法 选取2016年1月至2018年5月于青岛市黄岛区人民医院内分泌科住院的T2DM患者1225例,根据UACR水平分为正常白蛋白尿组(NAU,n=862)、微量白蛋白尿组(MAU,n=252)和大量白蛋白尿组(CAU,n=111),比较各组一般资料及Ins-Ab阳性率。结果 NAU、MAU、CAU组DM病程、SBP、UACR、hs-CRP、Ins-Ab和Ins-Ab阳性率依次升高(P<0.05)。Spearman相关分析显示,UACR与Ins-Ab呈正相关(P<0.05)。Logistic回归分析显示,校正DM病程、WHR、hs-CRP、TC、SBP、HbA1c等混杂因素后,Ins-Ab阳性是DKD的危险因素。结论 Ins-Ab水平与UACR密切相关,Ins-Ab阳性是DKD的危险因素。 Objective To explore the relationship between the level of insulin antibody(Ins-Ab)and urinary albumin/creatinine ratio(UACR)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 1225 T2DM patients hospitalized in The Department of Endocrinology,People’s Hospital of Huangdao were enrolled in this study from January 2016 to May 2018. All the participants were divided into three groups:normoalbuminuria group(NAU,n=862),microalbuminuria group(MAU,n=252)and macroalbuminuria group(CAU,n=111)according to the level of UACR. The general clinical characteristics and the Ins-Ab positive rate were compared among the three groups.ResultsThe duration of DM,SBP,UACR,hs-CRP,Ins-Ab and the positive rate of Ins-Ab increased gradually in NAU,MAU and CAU groups(P<0. 05). Spearman correlation analysis showed that Ins-Ab was positively correlated with UACR(P<0. 05). Logistic regression analysis showed that after adjusting duration of DM,WHR,hs-CRP,TC,SBP,HbA1c and other confounding factors,Ins-Ab positive was a risk factor for DKD.Conclusion Ins-Ab is closely related to UACR,and Ins-Ab positive is an independent risk factor for DKD.
