补肾祛湿汤联合刺络放血治疗腰椎间盘突出症效果分析

Effect of Bushen Qushi Decoction(补肾祛湿汤)Combined with Acupuncture and Bloodletting in Treatment of Lumbar Intervertebral Disc Herniation

目的分析补肾祛湿汤联合刺络放血治疗腰椎间盘突出症效果。方法选取104例腰椎间盘突出症患者进行研究,选取时间为2018年6月—2020年12月,将患者采用随机数字法分组。对照组52例给予牵引、康复训练及药物镇痛等常规对症治疗,观察组52例加用补肾祛湿汤联合刺络放血治疗。比较两组疗效,检测实验室指标,包括炎症因子[白细胞介素-10(IL-10)、白细胞介素-1β(L-1β)、白细胞介素-8(IL-8)]、前列腺素E2(PGE2)、磷脂酶A2(PLA2)、5-羟色胺(5-HT)的水平,并采用改良日本骨科协会腰痛评分表(M-JOA)和Oswestry功能障碍指数问卷表(ODI)评价肢体功能障碍程度。结果观察组中医证候总有效率高于对照组,差异具有统计学意义(P<0.05)。治疗前,两组实验室指标,包括炎症因子、PGE2、PLA2、5-HT比较,差异无统计学意义(P>0.05)。治疗后,观察组L-1β、IL-8、PGE2、PLA2、5-HT低于对照组,IL-10高于对照组(P<0.05)。治疗前,两组M-JOA评分和ODI评分比较,差异无统计学意义(P>0.05)。治疗后,观察组M-JOA评分和ODI评分均低于对照组(P<0.05)。结论补肾祛湿汤联合刺络放血治疗腰椎间盘突出症可减轻炎症反应,降低PGE2、PLA2、5-HT的表达水平,改善肢体功能障碍。

Objective To analyze the effect of Bushen Qushi Decoction(补肾祛湿汤)combined with needling collaterals and bloodletting in the treatment of lumbar disc protrusion.Methods A total of 104 patients with lumbar disc herniation were selected from June 2018 to December 2020.In the control group,52 cases were treated with traction,rehabilitation training and drug anal-gesia.In the observation group,52 cases were treated with Bushen Qushi Decoction and bloodletting.The efficacy of two groups,the levels of inflammatory factors[interleukin-10(IL-10),interleukin-1β(L-1),interleukin-8(IL-8)],the levels of prostaglandin E2(PGE2),phospholipase A2(PLA2)and serotonin(5-HT)were compared.The modified low back pain score(M-JOA)and Oswestry dysfunction index questionnaire(ODI)were used to evaluate the degree of limb dysfunction.Re-sults Compared with that of the control group,the total effective rate of TCM syndromes in the observation group was significantly higher(P<0.05).Before treatment,the levels of inflammatory factors,the levels of PGE2,PLA2 and 5-HT were not statistical-ly significant(P>0.05).The levels of L-1β,IL-8,PGE2,PLA2 and 5-HT in the observation group were lower than those in the control group and the IL-10 was higher than that in the control group(P<0.05).There was no significant difference in M-JOA score or ODI score between the two groups before treatment(P>0.05).The M-JOA score and ODI score of the obser-vation group were lower than those of the control group(P<0.05).Conclusion Bushen Qushi Decoction combined with needling collaterals and bloodletting can reduce the inflammatory reaction,decrease the expressions of PGE2,PLA2 and 5-HT and im-prove the limb dysfunction.
