闭合复位三枚克氏针固定治疗儿童肱骨髁上骨折的效果观察

Effect Observation of Closed Reduction and Fixation of Three Kirschner Wires in Treatment of Supracondylar Fractures of Humerus in Children

目的:探究闭合复位三枚克氏针固定治疗儿童肱骨髁上骨折的效果。方法:选取2019年6月-2020年6月衢州骨伤科医院收治的100例肱骨髁上骨折患儿作为研究对象,将其分为两组,各50例。对照组应用切开复位三枚克氏针固定治疗,试验组应用闭合复位三枚克氏针固定治疗。比较两组患儿治疗优良率、骨折愈合时间和并发症发生率。结果:试验组治疗优良率高于对照组,差异有统计学意义(P<0.05)。两组患儿均进行12个月随访,试验组患儿骨折在3~4个月愈合42例(84.0%),且无明显的并发症出现。对照组患儿3~4个月愈合23例(46.0%),有10例出现并发症,其中感染5例,骺损伤3例,肘内翻畸形1例,关节僵直1例,并发症发生率为20.0%。试验组骨折愈合率高于对照组,并发症发生率低于对照组,差异均有统计学意义(P<0.05)。结论:闭合复位三枚克氏针固定治疗儿童肱骨髁上骨折的效果良好,是一种理想、安全、可靠、有效的治疗方式,值得在临床上推广。

Objective:To explore the effect of closed reduction and fixation of three Kirschner wires in the treatment of supracondylar fractures of humerus in children.Methods:From June 2019 to June 2020,100 children with supracondylar fracture of humerus were treated in Quzhou Orthopedic Hospital as the study subjects and divided into two groups with 50 cases in each group.The control group was treated with open reduction and fixation of three Kirschner wires.The experimental group was treated with closed reduction and fixation of three Kirschner wires.The excellent and good rate,fracture healing time and complication rate of the two groups were compared.Results:In the experimental group,the excellent and good rate of treatment was higher than that in the control group,and the difference was statistically significant(P<0.05).The children in both groups were followed up for12 months.In the experimental group,the fractures of 42 children(84.0%)healed within 3~4 months,without obvious complications.In the control group,the fractures of 23 children(46.0%)healed within 3~4 months,and 10 children had complications,including 5 cases of infection,3 cases of epiphyseal injury,1 case of cubitus varus deformity,and 1 case of joint stiffness.The incidence of complications was 20.0%.The fracture healing rate of the experimental group was higher than that of the control group,and the incidence of complications of the experimental group was lower than that of the control group,with statistically significant differences(P<0.05).Conclusion:The effect of closed reduction and fixation of three Kirschner wires in the treatment of supracondylar fractures of humerus in children is significant.It is an ideal,safe,reliable and effective treatment method,which is worthy of clinical promotion.
