多学科团队延续护理在糖尿病足患者中的应用研究

Application of Multidisciplinary Team Continuous Care in Diabetic Foot Patients

目的:分析多学科团队(MDT)延续护理应用于糖尿病足患者中的效果。方法:选取2020年5月-2021年2月华中科技大学同济医学院附属协和医院收治的糖尿病足患者40例作为研究对象,采用随机数表法分为两组,各20例。对照组应用常规护理;观察组在常规护理基础上应用MDT延续护理。比较两组患者护理前后血糖指标水平和遵医行为。结果:护理前,两组患者的各项血糖指标水平比较,差异无统计学意义(P>0.05);护理后,两组患者的各项血糖指标水平相对比护理前明显更低,观察组的空腹血糖、餐后2 h血糖、糖化血红蛋白水平均显著低于对照组,差异有统计学意义(P<0.05)。观察组护理后的遵医用药、糖尿病饮食、运动治疗、自我检测、定期复查例数显著高于对照组,差异有统计学意义(P<0.05)。结论:在对糖尿病足患者进行护理期间,应用MDT延续护理,能够改善患者遵医行为,并降低其各项血糖指标水平,效果显著,可应用与推广。

Objective:To analyze the effect of multidisciplinary team(MDT)continuous care in diabetic foot patients.Methods:From May 2020 to February 2021,40 diabetic foot patients admitted to Union Hospital,Tongji Medical College,HuazhongUniversity of Science and Technology were selected as the study subjects and divided into two groups by random number tablemethod,with 20 cases in each group.The control group was given routine nursing;the observation group was given MDTcontinuous care on the basis of routine nursing.The blood glucose index levels and medical compliance behavior in the two groupsof patients before and after nursing were compared.Results:Before nursing,there was no significant difference in the levels ofvarious blood glucose indexes between the two groups of patients(P>0.05).After nursing,the levels of various blood glucoseindexes in the two groups were significantly lower than those before nursing,and the levels of fasting blood glucose,2 hpostprandial blood glucose and glycosylated hemoglobin in the observation group were significantly lower than those in the controlgroup,and the differences were statistically significant(P<0.05).The number of cases with compliant medication,diabetes diet,exercise treatment,self-examination,and regular re-examination after nursing in the observation group was significantly higherthan that in the control group,and the difference was statistically significant(P<0.05).Conclusion:During the nursing period ofdiabetic foot patients,the application of MDT continuous care can improve the patients’compliance behavior and reduce thelevels of various blood glucose indexes,with significant effect,it can be applied and promoted.
