摘要
陆灏教授临床基于“补脾升阳泻阴”原则治疗瘿病体现了李东垣“百病皆由脾胃衰而生”的学术观点。陆教授认为瘿病患者脾胃内伤的现象普遍存在,常因情志不畅、饮食不节、劳逸失调引起,病机在于中焦脾胃虚弱,中气不足,气机升降失调,日久阴血耗伤,虚火上浮。陆教授基于《脾胃论》建立瘿病辨治体系,灵活顾护脾胃元气,调畅气机,清泻阴火,交通心肾,调和阴阳,形成了自己的辨治特点,临床疗效颇佳。现结合临床医案,对陆教授治疗瘿病临证经验进行介绍。
Professor Lu Hao’s clinical treatment of goiter based on the principle of“invigorating spleen,promoting yang,and purging yin”embodies Li Dongyuan’s academic viewpoint that“all diseases are caused by spleen and stomach failure”.Professor Lu believes that the internal injury of spleen and stomach is common in patients with goiter,which is often caused by poor mood,poor diet,and imbalance between work and rest.The pathogenesis of goiter lies in spleen and stomach weakness in middle jiao,insufficiency of middle qi,imbalance of qi movement.After a long time of illness,the consumption of yin blood leads to upward floating of deficient fire.Professor Lu establishs a system of syndrome differentiation and treatment of goiter based on Piwei Lun.He pays attention to care for spleen and stomach vitality,regulate qi movement,purge yin fire,restoring normal coordination between heart and kidney,and harmonize yin and yang,which was flexible in clinical practice.His system of syndrome differentiation and treatment has achieved good clinical effect.This article introduces Professor Lu’s clinical experience in the treatment of goiter.
