乳腺肿块型浸润性导管癌X线征象与分子受体相关性研究

Correlation between X-ray features and molecular receptors in breast mass-type invasive ductal carcinoma

目的:探讨乳腺肿块型浸润性导管癌的X线影像征象及其与分子受体表达状态的相关性。方法:收纳行乳腺X线摄影检查的123例乳腺浸润性导管癌患者的相关数据,以病理结果为金标准,比较不同分子受体表达状态下乳腺腺体分型、肿块形态、边缘、密度的差异。结果:雌激素受体阳性者及阴性者在肿块形态及肿块边缘方面存在统计学差异(均P<0.05)。孕激素受体阳性组及阴性组患者的X线征象,在肿块边缘方面也存在统计学差异(均P<0.05),而在肿块形态方面无统计学差异(P>0.05)。人类表皮生长因子受体-2阳性组及阴性组患者X线征象的差异性主要体现为在肿块边缘方面,差异有统计学意义(P<0.05)。各受体不同表达情况在腺体分型、肿块密度上差异无统计学意义(均P>0.05)。Luminal A型及非Luminal A型患者的X线征象在肿块形态及肿块边缘方面,差异均有统计学意义(均P<0.05);在乳腺腺体分型及肿块密度方面,差异均无统计学意义(均P>0.05)。结论:乳腺肿块型浸润性导管癌的X线征象有助于分辨不同分子受体表达状态,有望为临床提供更精准的诊断及开展个性化治疗。

Objective:To investigate the correlation between X-ray features and the expression of molecular receptors in breast mass-type invasive ductal carcinoma.Methods:The data were collected on 123 patients with invasive ductal carcinoma of the breast who underwent mammography.Taking the pathological results as the gold standard,the breast glandular type,tumor shape,margin and density were compared under different molecular receptor expression states.Results:The estrogen receptor positive group and the negative group had significant differences in the shape and margin of the tumor(all P<0.05).There was also significant difference in the X-ray signs of the progesterone receptor positive group and the negative group in the margin of the mass(all P<0.05),but there was no significant difference in the shape of the mass(P>0.05).The differences of X-ray signs between the human epidermal growth factor receptor-2 positive group and the negative group were mainly reflected in the edge of the tumor,and the difference was significant(P<0.05).The different expressions of each receptor were no significant differences in glandular type and tumor density(all P>0.05).The X-ray signs of Luminal A and non-Luminal A patients were significantly different in terms of tumor shape and tumor margin(all P<0.05).There was no significant difference in breast glandular type and tumor density(all P>0.05).Conclusion:The X-ray features of breast mass invasive ductal carcinoma can helpful to distinguish the expression status of different molecular receptors,which is expected to provide more accurate diagnosis and personalized treatment for clinic.
