室性心动过速积分法在宽QRS波群心动过速鉴别诊断中的价值

摘要 目的 探讨室性心动过速积分法在宽QRS波群心动过速(WCT)鉴别诊断中的价值。方法 选取行心电图或24h动态心电图检查诊断为宽QRS波心动过速的患者100例进行回顾性分析,同时依据宽QRS波群室性心动过速积分法标准进行评分,并记录积分为0分、1分、2分、3分及>3分的病例数,计算出VT的发生率,以3分为节点,对积分≤2分与≥3分进行对比,统计鉴别诊断宽QRS波群心动过速的准确率、特异度、敏感性。结果 根据室速积分法,积分越高,室速的发生率越高;积分≥3分时诊断准确率(86. 00%)、特异度(86. 40%)及敏感性(86. 00%)显著高于积分≤2分时的诊断准确率(73. 30%)、特异度(73. 70%)及敏感性(72. 70%)。结论 应用室速积分法准确率、特异度及敏感性均较高,可对不同类型的室性心动过速快速做出诊断,具有重要的临床应用价值。 Objective To explore the value of ventricular tachycardia integral in differential diagnosis of tachycardia QRS complex.Methods A total of 100 patients with wide QRS complex tachycardia diagnosed by ECG or 24 h Holter were retrospectively analyzed,and the incidence rate of VT was calculated according to the standard of wide QRS complex ventricular tachycardia score at the same time,and the cases of the score of 0,1,2,3 were recorded,and the accuracy,specificity and sensitivity of integration≤2 and≥3 in differential diagnosis of wide QRS tachycardia were statistically analyzed.Results According to the method of ventricular velocity integration,the higher the integration,the higher the incidence of ventricular velocity;the diagnostic accuracy(86.00%),the specificity(86.40%)and the sensitivity(86.00%)were significantly higher in patients with integration≥3 than the diagnostic accuracy(73.30%)、the specificity(73.70%)and the sensitivity(72.70%)in patients with integration≤2.Conclusion The ventricular tachycardia integral method has high accuracy,specificity and sensitivity,and can be used for rapid diagnosis of different types of ventricular Tachycardia.
