Effect Evaluation of Family Doctor Service on Promoting Pregnant and Parturient Women System Management

目的:探讨家庭医生服务对促进孕产妇系统管理的效果。方法:选取2018年5月-2020年6月在北京市大兴区北臧村镇中心卫生院建立北京市母子健康档案的1654例孕产妇为此次研究对象,按照简单随机化法将其分为两组,各827例。对照组应用常规管理,观察组应用家庭医生签约服务。比较两组早孕建册率、产检依从性、自然分娩率、孕产妇并发症发生率、不良妊娠结局发生率及孕产妇满意度。结果:观察组早孕建册率、产检依从性及自然分娩率均高于对照组,差异具有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异具有统计学意义(P<0.05);观察组不良妊娠结局发生率低于对照组,差异具有统计学意义(P<0.05);观察组满意度高于对照组,差异具有统计学意义(P<0.05)。结论:家庭医生签约服务在孕产妇系统管理中能降低产妇生育风险,提高其早孕建册率、产检依从性及自然分娩率,降低不良妊娠结局发生率,在一定程度上可保证母婴健康,增加孕产妇的满意度,值得借鉴。

Objective:To explore the effect of family doctor service on promoting the system management of pregnant and parturient women.Methods:1654 pregnant women who established Beijing Maternal and child health files in the central health center of Beizang village,Daxing District,Beijing from May 2018 to June 2020 were selected as the research objects,and they were divided into the two groups according to the simple randomization method,with 827 cases in each group.The control group was given routine management,and the observation group was given the contracted service of family doctors.The rates of early pregnancy registration,obstetric inspection compliance,natural delivery rate,incidence of maternal complications,incidence of adverse pregnancy outcomes,and maternal satisfaction were compared between the two groups.Results:The rate of early pregnancy registration,obstetric inspection compliance,and natural delivery rate in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05);the incidence of maternal complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05);the incidence of adverse pregnancy outcomes in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05);the satisfaction in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Family doctor contract service in the system management of pregnant and parturient women can reduce maternal fertility risk,increase the rates of early pregnancy registration,obstetric inspection compliance and natural delivery,reduce the incidence of adverse pregnancy outcomes.It can ensure the health of mothers and babies,and increase the satisfaction of pregnant and parturient women,so it can serve as a reference.
