火针联合308 nm准分子激光治疗白癜风疗效分析

Efficacy Analysis of Fire Needle Combined with 308 nm Excimer Laser in the Treatment of Vitiligo

目的研究分析火针联合308 nm准分子激光治疗白癜风的临床疗效。方法选取2018年10月至2020年10月台前县夹河中心乡卫生院收治的85例白癜风患者(255处皮损)作为研究对象,按照随机数表法将其随机分为观察组42例(123处皮损)和对照组43例(132处皮损),观察组患者采用火针联合308 nm准分子激光治疗,对照组患者单纯采用308 nm准分子激光治疗,对比观察两组患者皮损面积、复色率、见效时间、临床疗效以及不良反应发生情况。结果治疗结束后,观察组患者皮损面积明显小于对照组(t=27.030,P<0.001),复色率明显高于对照组(Z=-17.691,P<0.001),见效时间明显短于对照组(Z=-6.554,P<0.001);观察组患者中皮损痊愈36处、显效46处、好转19处、无效22处,明显优于对照组患者的皮损痊愈25处、显效41处、好转32处、无效34处(Z=-2.623,P=0.009)。其间,观察组患者不良反应发生率为1.63%,与对照组患者的不良反应发生率0.76%无明显差异(χ^(2)=0.413,P=0.520)。结论与单纯应用308 nm准分子激光治疗相比,火针联合308 nm准分子激光可明显缩短白癜风皮损处肤色恢复时间,缩小皮损面积,提高复色率,疗效显著。

Objective To study the clinical efficacy of fire needle combined with 308 nm excimer laser in the treatment of vitiligo.Methods 85 patients with vitiligo(255 skin lesions),admitted to Jiahe Central Township Health Center from October 2018 to October 2020,were enrolled and divided,using the random number table,into study group(n=42,123 skin lesions)and control group(n=43,132 skin lesions).Patients in study group were treated with fire needle combined with 308 nm excimer laser,whereas patients in control group were treated with 308 nm excimer laser alone.Size of skin lesion,recoloration rate,effective time,clinical efficacy and occurrence of adverse reactions were compared between the two groups.Results After treatment,compared with control group,the size of skin lesions in study group was obviously smaller(t=27.030,P<0.001),the recoloration was markedly higher(Z=-17.691,P<0.001),and the effective time was significantly shorter(Z=-6.554,P<0.001)in study group;the healing condition of skin lesions were evaluated as healed in 36 lesions,markedly effective in 46 lesions,improved in 19 lesions and ineffective in 22 lesions in study group,which was significantly better than control group(Z=-2.623,P=0.009)-healed in 25 lesions,markedly effective in 41 lesions,improved in 32 lesions,and ineffective in 34 lesions.During the course of treatment,the incidence of adverse reactions was 1.63%in study group,which showed no significant difference compared with the corresponding0.76%in control group(χ^(2)=0.413,P=0.520).Conclusion Compared with the application of 308 nm excimer laser alone,the combined application of fire needle and 308 nm excimer laser in the treatment of vitiligo can significantly shorten the restoration time of skin pigment,decrease the size of skin lesions,and improve the recoloration rate,and the clinical efficacy was definite.
