止痒平肤液治疗晚期结肠癌西妥昔单抗痤疮样皮疹的临床研究

Clinical Study of Zhiyangpingfu Liquid on acne-like rash associated with cetuximab in advanced colon cancer

目的观察止痒平肤液治疗晚期结肠癌西妥昔单抗痤疮样皮疹的临床疗效。方法将80例晚期结肠癌西妥昔单抗痤疮样皮疹患者按照随机数字表法分为2组,对照组40例予西医常规治疗,治疗组40例予止痒平肤液治疗。2组均连续治疗14 d后统计疗效,比较2组治疗前后中医证候(包括皮损部位、皮损面积、皮疹颜色、皮损程度、瘙痒与疼痛、舌象、脉象)评分、皮肤反应严重程度分级及血清白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)水平变化。结果治疗组总有效率90.0%(36/40),对照组总有效率70.0%(28/40),治疗组疗效优于对照组(P<0.05)。与本组治疗前比较,2组治疗后中医证候皮损部位、皮损面积、皮疹颜色、皮损程度、瘙痒与疼痛、舌象、脉象评分均降低(P<0.05),且治疗组治疗后中医证候各项评分均低于对照组(P<0.05)。与本组治疗前比较,2组治疗后皮肤反应严重程度分级均有减轻(P<0.05),且治疗组治疗后皮肤反应严重程度分级轻于对照组(P<0.05)。与本组治疗前比较,2组治疗后血清IL-6及TNF-α水平均降低(P<0.05),且治疗组治疗后血清IL-6及TNF-α水平低于对照组(P<0.05)。结论止痒平肤液治疗晚期结肠癌西妥昔单抗痤疮样皮疹疗效确切,可有效改善患者中医证候表现,减轻皮肤反应严重程度,降低IL-6及TNF-α水平,抑制局部炎性反应。

Objective To explore the clinical efficacy of Zhiyangpingfu Liquid on acne-like rash associated with cetuximab in advanced colon cancer.Methods Totally 80 patients with advanced colon cancer patients with acne-like rash cetuximab were randomly divided into two groups,the control group(n=40)were treated with conventional Western medicine,and the treatment group(n=40)were treated with Zhiyang Pingfu Liquid.Treated for 14 days,and the curative effect was counted,and traditional Chinese medicine(TCM)syndrome scores(including skin lesion location,skin lesion area,skin rash color,skin lesion degree,itching and pain,tongue symptoms,pulse symptoms),the severity of skin reactions,serum interleukin 6(IL-6)and tumor necrosis factor alpha(TNF-α)levels were compared between groups before and after treatment.Results The total effective rates in the treatment group were better than those in the control group(90.0%[36/40]70.0%[28/40],(P<0.05);respectively).Compared with before treatment,the TCM syndrome scores were all decreased in both groups after treatment(P<0.05).which were lower in the treatment group than the control group(P<0.05).The frequency of skin reactions in the both groups was reduced after treatment(P<0.05),and which in the treatment group was lighter than the control group(P<0.05).Compared with that before treatment,the levels of serum IL-6 and TNF-α in the both groups were decreased after treatment(P<0.05),and which in the treatment group was common(P<0.05).Conclusion For acne-like rash associated with cetuximab in advanced colon cancer,the topical anti-itching Pingfu liquid can effectively improve the performance of TCM syndromes,reduce the severity of skin reactions,reduce the levels of IL-6 and TNF-α,and inhibit local inflammatory reactions.
