可溶性淋巴细胞活化基因-3、α-突触核蛋白水平对帕金森病诊断效能的临床研究 被引量:3

摘要 目的观察可溶性淋巴细胞活化基因-3(sLAG-3)、α-突触核蛋白(α-syn)水平对帕金森病(PD)的诊断效能。方法选取2020年12月至2021年4月本院神经内科收治的47例PD患者作为PD组,选择门诊同期年龄、性别与PD患者相匹配的36名体检者作为对照组。采用ELISA法检测血清sLAG-3、血浆α-syn、红细胞α-syn的表达水平,采用统一PD评分量表、Hoehn-Yahr分级、PD非运动症状评价量表(NMSS)评估PD患者临床症状,采用Spearman进行相关性分析,采用ROC曲线分析血清sLAG-3、红细胞α-syn、血浆α-syn水平对PD的诊断价值,二元Logistic回归分析α-syn与sLAG-3的关系。结果PD组血清sLAG-3、红细胞α-syn、血浆α-syn水平显著高于对照组(均P<0.05);Spearman分析显示,PD组血清sLAG-3水平与性别、NMSS相关(均P<0.05),ROC分析显示,血清sLAG-3水平曲线下面积为0.822,敏感度61.7%、特异度86.1%;红细胞α-syn曲线下面积为0.923,敏感度83.0%、特异度83.3%;血浆α-syn水平曲线下面积为0.916,敏感度97.7%、特异度66.7%。二元Logistic回归分析发现,血清sLAG-3与红细胞α-syn相关(OR=1.073,P<0.05)。结论sLAG-3、α-syn在PD组高表达;PD组血清sLAG-3与性别、NMSS相关;血清sLAG-3与红细胞α-syn相关,检测血清sLAG-3、红细胞α-syn和血浆α-syn水平对PD有一定诊断价值。 Objective To observe the diagnostic efficacy of soluble lymphocyte activating gene-3(sLAG-3)and α-synuclein( α-syn)in Parkinson’s disease(PD).Methods Forty-seven PD patients treated in the Department of Neurology of our hospital from December 2020 to April 2021 were selected as the PD group,and 36 physical examiners matched with PD patients in age and sex in the same period were selected as the control group.The expression levels of serum sLAG-3,plasma α-synuclein(plasma α-syn)and erythrocyte α-synuclein(erythrocyte α-syn)were detected by ELISA.The clinical symptoms of PD patients were evaluated by unified PD score scale,Hoehn Yahr and PD non motor symptom evaluation scale(NMSS).The correlation was analyzed by Spearman,ROC curve was used to analyze the diagnostic value of serum sLAG-3,erythrocyte α-syn and plasma α-syn levels in PD.The relationship between α-syn and sLAG-3 was analyzed by binary logistic regression.Results The levels of serum sLAG-3,erythrocyte α-syn and plasma α-syn in the PD group were significantly higher than those in the control group(allP<0.05).Spearman analysis showed that the serum sLAG-3 level in the PD group was correlated with gender and NMSS(allP<0.05).ROC analysis showed that the area under the curve of serum sLAG-3 level was 0.822,the sensitivity was 61.7%,and the specificity was 86.1%;the area under the curve of erythrocyte α-syn was 0.923,the sensitivity was 83.0%,and the specificity was 83.3%;the area under the curve of plasma α-syn level was 0.916,with a sensitivity of 97.7% and a specificity of 66.7%.Binary Logistic regression analysis found that serum LAG-3 was correlated with erythrocyte α-syn(OR=1.073,P<0.05).Conclusions Serum sLAG-3,erythrocyte α-syn and plasma α-syn are highly expressed in PD group;serum sLAG-3 level in PD group is correlated with gender and NMSS;serum sLAG-3 is correlated with erythrocyte α-syn levels.Detections of serum sLAG-3,erythrocyte α-syn and plasma α-syn levels have certain diagnostic value for PD.
