2010-2019年广西某综合医院HIV/AIDS病例晚发现情况及其影响因素分析

Analysis on late detection of HIV/AIDS cases and influencing factors in a general hospital, Guangxi,2010-2019

目的 了解2010-2019年广西某综合医院HIV/AIDS病例的晚发现状况及相关影响因素。方法 收集2010-2019年广西某综合医院门诊和住院新发现HIV/AIDS病例,利用SPSS 22.0软件进行描述性统计分析。结果 2010-2019年HIV/AIDS病例中CD4检测比例逐年上升,总体CD4细胞平均值为216.84个/μl。2 014例HIV/AIDS病例中,符合晚发现病例定义的1 114例,晚发现率达55.31%。多因素回归分析显示,年龄在26~55岁(O R=1.69,P<0.05)、有异性性行为史(O R=1.67,P<0.01)、出现HIV相关临床症状(O R=7.85,P<0.01)、内科系统就诊者(O R=2.09,P<0.01)晚发现的风险更高;而有同性接触史(O R=0.61,P<0.05)则是晚发现的保护因素。结论 广西某综合医院HIV/AIDS病例晚发现率高。既往有异性性行为史、内科系统就诊者、HIV相关的临床症状是晚发现的独立危险因素。

Objective To understand the characteristics and influencing factors for HIV/AIDS who were diagnosed late among the newly-confirmed cases in a general hospital in Guangxi during 2010 to 2019. Methods The newly-confirmed HIV/AIDS cases in the general hospital in Guangxi from 2010-2019 were collected for a statistical analysis with software SPSS22.0. Results The proportion of CD4 detection increased year by year, and the average value of overall CD4 cells was 216.84 cell/ul.Among the 2 014 cases, 1 114 met the definition of late detection cases, the late detection rate reached 55.31%.Multivariate regression analysis showed that the risk of late detection was higher in patients aged 26-55 years(O R=1.69,P<0.05),with a history of heterosexual behavior(O R=1.67,P<0.01),HIV related clinical symptoms(O R=7.85,P<0.01) and internal medicine system(O R=2.09,P<0.01);MSM(O R=0.61,P<0.05)was the protective factor of late detection. Conclusion The late detection rate of HIV/AIDS cases in a general hospital in Guangxi is high.Previous history of heterosexual behavior, patients in the internal medicine system and HIV related clinical symptoms are independent risk factors for late detection.
