多学科协作健康管理模式在健康管理中心中的应用

目的 探讨多学科协作健康管理模式在健康管理中心中的应用效果。方法 选取2019年1月-12月在笔者所在医院健康管理中心进行体检,体检结果提示有脂肪肝、高血压、高血脂、高血糖之一的1 600例患者为研究对象。采用随机数字表法将其分为普通流程组和多学科协作健康管理组,各800例。普通流程组按常规的健康管理流程进行干预,多学科协作组则采用多学科协作的慢性病健康管理模式进行干预。比较2组就诊率、患者满意率以及干预前后2组脂肪肝、高血压、高血脂、高血糖的患病人数。结果 多学科健康管理组就诊率及患者满意率均高于普通流程组(χ^(2)=64.825,P<0.001;χ^(2)=36.471,P<0.001)。干预1年后多学科健康管理组脂肪肝、高血压、高血脂、高血糖人数均少于普通流程组(P<0.001)。结论 多学科协作慢性病健康管理护理模式有利于提高患者就诊率和满意率,促进患者主动健康管理的意识,进而促进疾病的健康转归。

Objective To explore the effect of applying the multidisciplinary health management in the health management center.Methods Totally 1 600 patients with fatty liver,hypertension,hyperlipidemia or hyperglycemia at the health management center of our hospital in year 2019,were divided into a control group and a multidisciplinary health management group,each of 800,according to a random number table.The control group was given routine health management,while the multidisciplinary health management group was provided with chronic disease management based on the multidisciplinary collaboration.The visiting rate and patients′satisfaction were compared between the two groups.Meanwhile,the number of patients suffering from fatty liver,hypertension,hyperglycemia or hyperlipidemia before and after the intervention were recorded and compared.Results The satisfaction and visiting rate of the multidisciplinary health management group were significantly higher than those of the control group (χ^(2)=36.471,P <0.001;χ^(2)=64.825,P <0.001).One year after the intervention,the numbers of patients with fatty liver,hypertension,hyperglycemia or hyperlipidemia of the multidisciplinary health management group were significantly smaller than that of the control group.Conclusion The multidisciplinary health management can promote the visiting rate and satisfaction of patients,initiate their awareness of health management,promoting their health condition.
