女性慢性肾功能衰竭行血液透析患者生活质量的影响因素分析

Influencing factors of quality of life in female chronic renal failure patients undergoing hemodialysis

目的探讨女性慢性肾功能衰竭行血液透析(HD)患者生活质量的影响因素。方法选取2017年1月—2020年12月湖州市第一人民医院收治的80例女性慢性肾功能衰竭行HD患者为研究对象,所有患者均接受生活质量量表(SF-36)评估,调查患者一般资料,分析慢性肾功能衰竭行HD患者生活质量的影响因素。结果80例慢性肾功能衰竭行HD患者,平均SF-36评分为(78.60±6.48)分。不同婚姻状况、睡眠质量、社会支持及自我效能患者SF-36评分比较,差异均有统计学意义(均P<0.05)。经多元线性回归分析得到方程:Y=80.901-1.989X_(1)-2.640X_(2)-2.336X_(3)-2.171X_(4),X_(1)=婚姻状况,X_(2)=睡眠质量,X_(3)=社会支持,X_(4)=自我效能。结果显示,已婚、睡眠质量良好、有社会支持及中高水平自我效能是慢性肾功能衰竭行HD患者生活质量的影响因素(t=2.407、3.674、3.195及3.241,均P<0.05)。结论未婚、睡眠质量差、社会支持差及低水平自我效能是慢性肾功能衰竭行HD患者生活质量的影响因素,临床可根据患者存在的风险因素,进行针对性干预,提升患者生活质量。

Objective To explore the factors influencing the quality of life of female patients with chronic renal failure undergoing he-modialysis(HD).Methods A total of 80 female HD patients with chronic renal failure admitted to the First People's Hospital of Huzhou city from January 2017 to December 2020 were selected as the research object.All patients were evaluated by the Quality of Life Scale(SF-36),the general data of the patients were investigated,and the factors affecting the quality of life of patients with chronic renal failure and HD were analyzed.Results The average SF-36 score of 80 patients with chronic renal failure undergoing HD was(78.60±6.48)points;there were significant differences in the SF-36 scores of patients with different marital status,sleep quality,social support,and self-efficacy(P<0.05);the equation obtained by multiple linear regression analysis:Y=80.901-1.989X_(1)-2.640X_(2)-2.336X_(3)-2.171X_(4),X_(1)=marital status,X_(2)=sleep quality,X_(3)=social support,X_(4)=self-efficacy,the results show that,Being married,having good sleep quality,having so-cial support,and medium-high level of self-efficacy were the influencing factors of quality of life in patients with chronic renal failure and HD(t=2.407,3.674,3.195 and 3.241,all P<0.05).Conclusion Unmarried,poor sleep quality,poor social support,and low level of self-efficacy are factors that affect the quality of life of patients with chronic renal failure and HD.Clinically,targeted interventions can be made to improve the quality of life of patients based on the patient's existing risk factors.
