Clinical pharmacist participated in the medication analysis of a patient with atrial fibrillation who took warfarin overdose,not only to provide opinions on the treatment of patients with warfarin overdose and the choice of new anticoagulants,but also to provided pharmaceutical care for patients using rivaroxaban-a new oral anticoagulant drug.Clinical pharmacist improved the medication compliance for the patient,and escorted the patient for clinical safety and rational drug use in this treatment.
