经颅直流电刺激与强化肌力训练对帕金森非运动性障碍患者的影响

Effect of Transcranial Direct Current Stimulation and Intensive Muscle Strength Training on Non-motor Disorders in Parkinson′s Patients

摘要 目的:探讨经颅直流电刺激与强化肌力训练对帕金森非运动性障碍患者的影响。方法:选取2018年10月1日~2020年10月31日收治的134例帕金森非运动性障碍患者,随机分为对照组和研究组各67例,对照组给予假性刺激和常规训练,研究组给予经颅直流电刺激与强化肌力训练;比较两组日常活动能力[采用Barthel指数(BI)]、下肢运动功能(采用Fugl-Meyer运动功能评定量表)、平衡功能(采用Fugl-Meyer平衡功能量表)及认知功能[采用蒙特利认知评估量表(MoCA)]。结果:研究组BI、Fugl-Meyer下肢运动功能评定量表评分、Fugl-Meyer平衡功能量表评分均高于对照组(P<0.01),MoCA中定向能力、视知觉、空间知觉、动作运用、视运动组织、思维操作得分均高于对照组(P<0.01)。结论:在帕金森非运动性障碍患者中应用经颅直流电刺激与强化肌力训练效果明显,在改善患者下肢功能和平衡功能的同时,提高日常生活能力,改善认知功能,提高患者生活质量。 Objective:To explore the effect of transcranial direct current stimulation and intensive muscle strength training in Parkinson′s patients with non-motor disorders.Methods:During October 1,2018 to October 31,2020,134 patients with Parkinson′s disease with non-motor disorder were included and randomly divided into the control group and the study group,with 67 cases in each group.The control group was given pseudo stimulation and routine training,while the study group was given transcranial direct current stimulation and intensive muscle strength training.Comparison was made between the two groups on daily activity ability by Barthel index(BI),lower limb motor function by the Fugl-Meyer movement function rating scale,balance function by the Fugl-Meyer balance function scale and cognitive function by the Montreal cognitive assessment scale(MoCA).Results:The BI scores,Fugl-Meyer lower limb motor function and balance function in the study group were higher than those in the control group(P<0.01).The MoCA scores of orientation ability,visual perception,spatial perception,movement application,visual and motor organization and thinking operation in the study group were higher than those in the control group(P<0.01).Conclusion:Application of transcranial direct current stimulation and intensive muscle strength training is effective in Parkinson′s patients with non-motor disorders,which can improve lower limb function and balance function,improve daily living ability,improve cognitive function and improve their quality of life.
