老年阻塞性睡眠呼吸暂停低通气综合征与高血压的相关性分析

Correlation Analysis of Obstructive Sleep Apnea Hypopnea Syndrome and Hypertension in Elderly Patients

目的:分析老年阻塞性睡眠呼吸暂停低通气综合征(OSAHS)与高血压的相关性。方法:选取2019年1月至2020年1月北京市延庆区医院呼吸内科收治的老年鼾症患者96例作为研究对象,对所有患者实施多导睡眠仪检查,基于睡眠呼吸暂停低通气指数以及夜间最低血氧饱和度数值划分为4组,分别是单一鼾症组、OSAHS轻度组、OSAHS中度组与OSAHS重度组,每组24例,分析老年OSAHS与高血压之间的相关性。结果:OSAHS轻度组、OSAHS中度组与OSAHS重度组患者共计72例,伴随高血压患者有38例,占比52.78%,单一鼾症患者24例,伴随高血压患者有3例,占比12.50%,单一鼾症组患者伴随高血压的发生率小于鼾症伴随不同程度的呼吸暂停低通气综合征组,各组间比较差异均有统计学意义(均P<0.05)。经晨起多导睡眠仪监测患者血压,晨起单一鼾症组患者收缩压数值平均(130.36±8.12)mm Hg(1 mm Hg=0.133 kPa)、舒张压数值平均(71.72±8.35)mm Hg,鼾症伴随不同程度的OSAHS患者收缩压平均数值为(153.60±9.31)mm Hg、舒张压平均数值为(96.99±9.64)mm Hg,均大于单一鼾症患者组,各组间比较差异均有统计学意义(均P<0.05)。结论:老年OSAHS与高血压存在一定关联性,二者的临床表现密切相关,应该引起医护工作者的重视,在临床的治疗与护理过程中,要时刻关注老年患者血压的变化,及时了解老年患者身体健康情况,进而更好地帮助老年OSAHS患者提高睡眠质量与生命质量。

Objective:To investigate the correlation between obstructive sleep apnea hypopnea syndrome(OSAHS)and hypertension in the elderly Patients.Methods:A total of 96 elderly patients with snoring from January 2019 to January 2020 in the department of Respiratory Medicine,Beijing Yanqing District Hospital were selected as the research subjects.All patients underwent polysomnography examination and were divided into 4 groups based on sleep apnea hypopnea index and minimum blood oxygen saturation at night.There were 24 patients in each group,including single snoring group,mild OSAHS group,moderate OSAHS group and severe OSAHS group,to analyze the correlation between OSAHS and hypertension in the elderly.Results:There were 72 patients in mild OSAHS group,moderate OSAHS group and severe OSAHS group,38 patients with hypertension(52.78%),24 patients with single snoring(12.50%)and 3 patients with hypertension(12.50%).The incidence of hypertension in patients with single snoring was lower than that in patients with different degrees of apnea hypopnea syndrome,and the differences were statistically significant(P s<0.05).The mean systolic blood pressure and diastolic blood pressure in the single snoring group were(130.36±8.12)mm Hg and(71.72±8.35)mm Hg respectively.The average systolic blood pressure and diastolic blood pressure were(153.60±9.31)mm Hg and(96.99±9.64)mm Hg in patients with morning snore accompanied by different degrees of apnea hypopnea syndrome,which were higher than those in patients with single snoring,with statistical significance among all groups(P s<0.05).Conclusion:OSAHS is associated with hypertension in the elderly patients.The clinical manifestations of OSAHS and hypertension are closely related,which should arouse the attention of medical workers.In the process of clinical treatment and nursing,it is necessary to pay attention to the changes of blood pressure of elderly patients in order to better help elderly patients with OSAHS to improve sleep quality and quality of life.
