坤泰胶囊联合戊酸雌二醇治疗卵巢早衰的疗效及其对患者卵巢血流动力学及性激素水平的影响

Effect of Kuntai capsule combined with estradiol valerate on premature ovarian failure and its effect on ovarian hemodynamics and sex hormone level

目的:探析坤泰胶囊联合戊酸雌二醇治疗卵巢早衰(POF)的临床疗效及其对患者卵巢血流动力学及性激素水平的影响。方法:本研究纳入2018年1月至2020年12月经本院妇科确诊的80例POF患者作为研究对象,按随机数字表法分为对照组40例患者,接受戊酸雌二醇治疗方案;另40例患者作为研究组,接受坤泰胶囊联合戊酸雌二醇联合治疗方案;比较两组患者治疗后的临床有效率及治疗前后卵巢螺旋动脉搏动指数(PI)、阻力指数(RI)、黄体生成素(LH)、雌二醇(E_2)和促卵泡生成素(FSH)的变化情况、卵巢功能、不良反应。结果:两组患者治疗前的卵巢血流动力学指标与性激素水平、卵巢功能比较,差异无统计学意义(P>0.05);两组治疗后不良反应无差异(P>0.05);两组经过治疗后的PI、RI、LH、FSH水平均降低,E_2水平均升高,且研究组PI、RI、LH、FSH水平变化较对照组更低,E_2水平变化较对照组更高,研究组治疗有效率高于对照组,治疗后研究组子宫内膜厚度厚于对照组,卵巢体积大于对照组,差异有统计学意义(P<0.05)。结论:采用坤泰胶囊联合戊酸雌二醇治疗POF,能够改善患者卵巢血流动力学与性激素水平,改善卵巢功能,整体疗效显著且安全性较高,此种中西药协同治疗的方法值得临床推广应用。

Objective:To explore the clinical efficacy of Kuntai capsule combined with estradiol valerate in the treatment of premature ovarian failure(POF)and its effects on ovarian hemodynamics and sex hormone levels.Methods:This study included 80 patients with POF diagnosed by gynecology department of our hospital from January 2018 to December 2020 as the research object,and the random number table method was used as the grouping method of this trial.After grouping,there were 40 patients in the control group and received estradiol valerate treatment.Another 40 patients as the experimental received Kuntai capsule combined with estradiol valerate.The clinical effective rate,ovarian spiral artery PI,RI,LH,E2 and FSH,ovarian function and adverse reactions were compared between the two groups before and after treatment.Results:There was no significant difference in ovarian hemodynamics,sex hormone level and ovarian function between the two groups before treatment(P>0.05).There was no significant difference in adverse reactions between the two groups after treatment(P>0.05).After treatment,the levels of PI,RI,LH and FSH in the study group and the control group decreased,and the levels of E2 increased.The changes of PI,RI,LH and FSH in the study group were lower than those in the control group,and the changes of E2 level were higher than those in the control group.Moreover,the treatment efficiency of the study group was higher than that in the control group.After treatment,the thickness of endometrium in the study group was thicker than that in the control group,and Compared with the control group,the ovarian volume of the study group was larger(P<0.05).Conclusion:The use of Kuntai capsules combined with estradiol valerate in the treatment of POF can improve ovarian hemodynamics and sex hormone levels,and improve ovarian function.The overall curative effect is significant and the safety is high.This Chinese and Western medicine synergistic treatment method is worthy of clinical application.
