亚甲基四氢叶酸还原酶基因多态性与脂代谢和肝脏脂肪含量的关系

摘要 目的探讨亚甲基四氢叶酸还原酶(MTHFR)基因C677T位点多态性与脂代谢、肝脏脂肪含量的关系。方法纳入238例广西南宁市壮汉族居民为研究对象,检测其MTHFR基因型、相关代谢指标、肝脏脂肪含量,并记录血压、体质指数等指标。根据MTHFR基因型检测结果,将携带MTHFR基因C677T位点CC基因型的研究对象作为野生组,将携带CT基因型或TT基因型的研究对象作为突变组。比较两组研究对象上述各指标的差异,并采用Logistic回归模型分析与MTHFR基因突变相关的因素。结果携带MTHFR基因C677T位点CC基因型、CT基因型、TT基因型各144例(60.5%)、81例(34.0%)、13例(5.5%)。与野生组相比,突变组的同型半胱氨酸(Hcy)、空腹胰岛素、三酰甘油水平较高,HDL-C水平较低(均P<0.05);两组的脂肪肝发生率、肝脏脂肪含量、收缩压、舒张压、体质指数、腰围、臀围,以及空腹血糖、LDL-C、尿酸、血管紧张素转换酶、血沉、维生素D 2、维生素D 3水平比较,差异均无统计学意义(均P>0.05)。Logistic回归分析结果显示血清Hcy、HDL-C水平与MTHFR基因突变相关(均P<0.05)。结论MTHFR基因C677T位点突变可伴随脂代谢紊乱,但对肝脏脂肪含量无明显影响。 Objective To explore the relation of methylenetetrahydrofolate reductase(MTHFR)gene C677T locus polymorphism with lipid metabolism and liver fat content.Methods A total of 238 Zhuang or Han inhabitants of Nanning City,Guangxi were enrolled as the research subjects.Their MTHFR genotype,related metabolism indices,and liver fat content were detected,and the indicators of pressure,body mass index etc.were recorded.The research subjects carrying MTHFR gene C677T locus CC genotype were seen as the wild group,whereas the research subjects carrying CT genotype or TT genotype were taken as the mutation group.The differences in various indices as above were compared between the two groups;furthermore,the Logistic regression model was used to analyze factors related to MTHFR gene mutation.Results The research subjects carrying MTHFR gene C677T locus CC genotype,CT genotype,TT genotype were 144 cases(60.5%),81 cases(34.3%),and 13 cases(5.5%),respectively.Compared with the wild group,the mutation group exhibited higher levels of homocysteine(Hcy),fasting insulin,triacylglycerol,while a lower level of HDL-C(all P<0.05).No statistically significant difference was found in the incidence rate of fatty liver,liver fat content,systolic pressure,diastolic pressure,body mass index,waistline,hipline,as well as the levels of fasting blood glucose,LDL-C,uric acid,angiotensin converting enzyme,erythrocyte sedimentation,vitamin D 2,and vitamin D 3(all P>0.05).The results of Logistic regression analysis revealed that serum Hcy,HDL-C levels were related to MTHFR gene mutation(all P<0.05).Conclusion The mutation of MTHFR gene C677T locus may accompany by lipometabolic disturbance;however,it has no significant effect on liver fat content.
