输尿管钬激光碎石术治疗输尿管中下段结石的效果

Effect of Ureteral Holmium Laser Lithotripsy in the Treatment of Middle and Lower Ureteral Calculi

目的:比较分析输尿管中下段结石患者应用输尿管钬激光碎石术(URSHL)以及体外冲击碎石术(ESWL)治疗的临床效果。方法:选取天津市蓟州区人民医院2019年3月至2020年8月间收治的输尿管中下段结石患者为研究对象,共计86例。根据手术方式不同将所有患者分为2组:URSHL组(n=43)和ESWL组(n=43)。比较两组手术时间、术后疼痛缓解率、结石清除率、并发症发生情况。结果:URSHL组手术时间较ESWL组降低,术后疼痛缓解率、总体结石清除率较ESWL组明显升高(P<0.05),尤其是直径>1cm以上的结石URSHL清除效果更好(P<0.05)。在术后并发症发生率方面,两组之间并无统计学差异(P>0.05)。结论:输尿管中下段结石患者应用URSHL的手术时间短,术后疼痛缓解率、总体结石清除率高,且术后并发症发生较少,值得临床应用。

Objective:To compare and analyze the clinical effects of ureteral holmium laser lithotripsy(urshl)and extracorporeal shock lithotripsy(ESWL)in the treatment of middle and lower ureteral calculi.Methods:86 patients with middle and lower ureteral calculi treated in Tianjin Jizhou District People's Hospital from March 2019 to August 2020 were selected as the research objects.All patients were divided into two groups according to different surgical methods:urshl group(n=43)and ESWL group(n=43).The operation time,postoperative pain relief rate,stone clearance rate and complications were compared between the two groups.Results:The operation time in urshl group was lower than that in ESWL group,and the postoperative pain relief rate and overall stone clearance rate were significantly higher than those in ESWL group(P<0.05),especially for stones with a diameter greater than 1cm,the urshl removal effect was better(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion:Urshl in patients with middle and lower ureteral calculi has short operation time,high postoperative pain relief rate and overall stone clearance rate,and less postoperative complications,which is worthy of clinical application.
