摘要
目的:研究超声辅助机械通气在慢性阻塞性肺疾病急性加重期中的应用价值。方法:选取2020年2月-2021年2月曲靖市第二人民医院收治的100例慢性阻塞性肺疾病急性加重期患者为研究对象,随机分为试验组、对照组,各50例,试验组采用超声辅助机械通气,对照组采用常规机械通气,观察两组临床治疗效果及对血气指标影响。结果:两组患者机械通气前动脉血气指标比较,差异无统计学意义(P>0.05);机械通气后,试验组动脉血二氧化碳分压(PaCO_(2))、动脉血氧分压(PaO2)和pH值指标优于对照组,差异具有统计学意义(P<0.05)。两组患者机械通气前脑钠肽(BNP)水平比较,差异无统计学意义(P>0.05);机械通气后,试验组血浆BNP水平优于对照组,差异具有统计学意义(P<0.05)。机械通气后,试验组总有效率高于对照组,差异具有统计学意义(P<0.05)。试验组生活质量量表中心理健康、生理健康、社会功能、角色功能总体精神得分均高于对照组患者,差异有统计学意义(P<0.05)。试验组持续通气舒适度高于对照组,差异具有统计学意义(P<0.05)。结论:慢性阻塞性肺疾病急性加重期患者机械通气治疗中采用超声辅助治疗效果确切,可进一步提升总体疗效。
Objective:To study the application value of ultrasound-assisted mechanical ventilation during acute exacerbation of chronic obstructive pulmonary disease.Methods:A total of 100 patients with chronic obstructive pulmonary disease at acute exacerbation stage who were admitted to Qujing Second People's Hospital from February 2020 to February 2021 were selected as the study subjects.They were randomly divided into experimental group and control group with 50 cases in each group.The experimental group was given ultrasound-assisted mechanical ventilation and the control group was given conventional mechanical ventilation.The clinical treatment effect and influence on blood gas indexes of the two groups were observed.Results:There was no significant difference in arterial blood gas indexes between the two groups before mechanical ventilation(P>0.05);after mechanical ventilation,the arterial blood carbon dioxide partial pressure(PaCO_(2)),arterial blood oxygen partial pressure(PaO2)and pH value indexes of the experimental group were better than those of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the level of brain natriuretic peptide(BNP)between the two groups before mechanical ventilation(P>0.05);after mechanical ventilation,the plasma BNP level in the experimental group was better than that in the control group,with a statistically significant difference(P<0.05).After mechanical ventilation,the total effective rate in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).The mental health,physical health,social function,role function and overall mental score in the quality of life scale of the experimental group were all higher than those of the control group,and the difference was statistically significant(P<0.05).The continuous ventilation comfort of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Ultrasound-assisted mechanical ventilation treatment of patients with chronic obstructive pulmonary disease at acute exacerbation stage has effective effect and can further improve the overall efficacy.
