加味贝母瓜蒌散治疗老年社区获得性肺炎(痰热壅肺证)的临床效果

Clinical Effect of Modified Beimu Gualou Power in Treating Senile Community Acquired Pneumonia with Syndrome of Phlegm-Heat Obstructing Lung

目的:观察加味贝母瓜蒌散治疗老年社区获得性肺炎(CAP)(痰热壅肺证)患者的临床效果。方法:选取2019年5月-2021年12月广西中医药大学第一附属医院呼吸内科收治的68例老年CAP患者作为研究对象,采用随机数表法分为观察组和对照组,各34例。对照组给予常规西医治疗,观察组在对照组基础上服用加味贝母瓜蒌散,比较两组患者治疗前后外周血清白细胞计数(WBC)、中性粒细胞百分比(NE%)、C反应蛋白(CRP)、降钙素原(PCT)、中医证候积分及临床肺部感染评分(CPIS)。结果:治疗后,两组WBC、NE%、CRP、PCT、中医证候积分及CPIS评分均低于治疗前,差异有统计学意义(P<0.05);治疗后,观察组WBC、NE%、CRP、PCT、中医证候积分及CPIS评分均低于对照组,差异有统计学意义(P<0.05)。结论:对于痰热壅肺型老年CAP患者,在常规西医治疗的基础上加用贝母瓜蒌散,可降低患者外周血炎性反应,改善临床症状,值得临床推广应用。

Objective:To observe the clinical effect of modified Beimu Gualou Powder in the treatment of elderly patients with community acquired pneumonia(CAP)(phlegm-heat obstructing lung syndrome).Methods:From May 2019 to December 2021,68 elderly patients with CAP who were admitted to the Department of Respiratory Medicine,The First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine were selected as the research objects,and divided into an observation group and a control group by random number table method,with 34 cases in each group.The control group was given conventional western medicine treatment,and the observation group was given modified Beimu Gualou Powder on the basis of the control group.The peripheral serum white blood cell count(WBC),neutrophil percentage(NE%),C-reactive protein(CRP),procalcitonin(PCT),TCM syndrome score and clinical pulmonary infection score(CPIS)were compared in two groups before and after treatment.Results:After treatment,the WBC,NE%,CRP,PCT,TCM symptom score and CPIS score of the two groups were all lower than those before treatment,and the difference was statistically significant(P<0.05).After treatment,the,WBC,NE%,CRP,PCT,TCM symptom score and CPIS score were lower than the control group,the difference was statistically significant(P<0.05).Conclusion:For elderly CAP patients with phlegm-heat obstructing lung syndrome,on the basis of conventional western medicine treatment,the addition of Beimu Gualou Powder can reduce the inflammatory reaction of peripheral blood and improve clinical symptoms,which is worthy of clinical application.
