首发脑卒中患者住院期间的家庭应对:基于家庭优势视角的质性研究

Family Coping of the First-episode Stroke Patients During Hospitalization:A Qualitative Study from the Perspective of Family Strength

目的从家庭优势视角出发,了解住院期间首发脑卒中患者家庭的应对型态,剖析其家庭优势,为护理人员提供以家庭为中心的干预奠定基础。方法采用描述性质性研究,通过便利抽样法和目的抽样法对上海市某三级甲等医院11例住院患者家庭进行半结构式深入访谈,采用内容分析法对访谈资料进行整理分析。结果家庭优势视角下住院期间脑卒中患者家庭的应对提取出2类主题和7个亚主题:患者家庭的应对型态(体现不同的家庭价值观,存在不同的家庭沟通模式,存在不同的家庭组织形式);应对中的患者家庭优势(正向的家庭信念,清晰、开放的沟通,充足的家庭资源,家庭组织的凝聚性和灵活性)。结论住院期间脑卒中患者家庭呈现出多种应对型态,家庭优势视角体现出患者的家庭弹性;护士可通过引导、强化脑卒中患者家庭优势来增强其家庭的有效应对。

Objective To understand the coping styles of the families of first-episode stroke patients during hospitalization and analyze their family strength from the perspective of family strength,so as to lay the foundation for family-centered interventions for caregivers.Methods a descriptive research design was applied,11 stroke inpatients’families selected by the convenience sampling method from a tertiary A hospital in Shanghai,were conducted semi-structured in-depth interviews,the interview data were sorted and analyzed by content analysis.Results From the perspective of family strength,two themes and seven sub-themes were extracted from the family coping of stroke patients during hospitalization:the coping styles of patients’families(reflecting different family values,different family communication modes and different family organization forms),and the family strength in coping(positive family belief,clear and open communication,sufficient family resources,cohesion and flexibility of family organization).Conclusions During hospitalization,the families of stroke patients show a variety of coping styles,and the perspective of family strength reflects their family resilience.Nurses can enhance the effective coping of their families by strengthening their family strength and resilience.
