ERCC1表达及基因多态性与口腔鳞状细胞癌患者临床特征及生存期的关系

Relationship between ERCC1 Expression,Polymorphisms and Clinical Characteristics,Survival Time in Patients with Oral Squamous Cell Carcinoma

目的分析切除修复交叉互补基因1(ERCC1)表达及基因多态性与口腔鳞状细胞癌(OSCC)患者临床特征及生存期的关系。方法选取口腔鳞状细胞癌患者62例作为观察对象,手术切取OSCC患者新鲜癌组织和癌旁组织,采用免疫组化检测ERCC1蛋白在癌组织和癌旁组织中的表达。采用质谱法分析ERCC1 C8092A位点多态性在OSCC患者中分布情况。采用Kaplan-Meier法描绘患者生存曲线。结果口腔鳞状癌组织中ERCC1阳性表达45例(72.58%),高于癌旁组织中ERCC1阳性表达17例(27.42%)(P<0.05)。45例ERCC1阳性表达患者36个月的中位生存期为27.80个月,17例ERCC1阴性表达患者36个月中位生存期为30.00个月,差异无统计学意义(P>0.05)。癌组织ERCC1表达阳性与肿瘤淋巴结转移、临床分期有关(P<0.05),与患者性别、年龄、分化程度、肿瘤分期和肿瘤大小无关(P>0.05)。癌组织ERCC1阳性表达患者CA基因型分布最多(60.00%),ERCC1阴性表达患者CC基因型分布最多(62.50%),ERCC1阳性表达与阴性表达患者C8092A基因型分布差异具有统计学意义(P<0.05)。基因型CC和CA患者中位生存期分别为31.10个月和32.80个月,均长于基因型AA患者的21.80个月(P<0.05)。结论ERCC1基因表达与OSCC患者生存期无显著关系,其基因C8092A位点多态性与患者生存期有关,重点分析C8092A位点对改善患者预后意义重大。

Objective To analyze the relationship between the expression and polymorphisms of excision repair cross complementing 1(ERCC1)and clinical characteristics,survival time in patients with oral squamous cell carcinoma(OSCC).Methods A total of 62 patients with oral squamous cell carcinoma were selected.The fresh cancer tissues and para-carcinoma tissues in OSCC patients were collected,and expression of ERCC1 protein in the two tissues was detected by immunohistochemistry.The polymorphisms of ERCC1 at C8092A locus were analyzed by mass-spectrography.The survival curves of patients were analyzed by Kaplan-Meier.Results The positive expression of ERCC1 in OSCC tissues was higher in 45 cases(72.58%)than in para-carcinoma tissues[17 cases(27.42%)](P<0.05).The median survival time in cancer tissues and para-carcinoma tissues was 27.80 months and 30.00 months,respectively(P>0.05).ERCC1 positive in cancer tissue was related to lymph node metastasis and clinical staging(P<0.05),while not related to gender,age,differentiation degree,tumor staging or tumor size(P>0.05).The frequency of CA genotype was the highest(60.00%)in patients with ERCC1 positive,while frequency of CC genotype was the highest(62.50%)in patients with ERCC1 negative.The differences in the distribution of ERCC1 genotypes at C8092A locus between patients with ERCC1 positive and ERCC1 negative were statistically significant(P<0.05).The median survival time in patients with CC and CA genotypes was 31.10 months and 32.80 months,longer than that with AA genotype(21.80 months)(P<0.05).Conclusion There is no significant relationship between ERCC1 gene and survival time in OSCC patients.The polymorphisms of ERCC1 at C8092A locus are related to survival time.Selective analysis of polymorphisms at C8092A locus is of great significance in improving prognosis of patients.
