鼻内镜治疗鼻腔鼻窦内翻性乳头状瘤的临床效果及复发因素分析

Clinical Effect and Recurrence Factor Analysis of Nasal Sinus Overturned Papilloma

目的探讨鼻内镜治疗鼻腔鼻窦内翻性乳头状瘤的临床效果及复发因素。方法选取鼻腔鼻窦内翻性乳头状瘤患者120例,按随机分为研究组(60例)和对照组(60例)。对照组行传统鼻腔解剖术治疗,研究组行鼻内镜下鼻腔解剖术治疗。对比两组手术相关指标(手术时间、术中出血量、手术成功率,VAS评分、复发率),采用logistics回归模型对患者复发影响因素进行分析。结果研究组手术时间、术中出血量和治疗后VAS评分显著低于对照组(P<0.05),而两组手术成功率无显著差异(P>0.05);研究组复发率显著低于对照组(8.34%vs 25.00%)(P<0.05);多因素logistics回归结果显示,鼻部手术史、肿瘤生发中心和临床分级为独立危险因素(P<0.05)。结论鼻内镜下解剖术治疗鼻窦内翻性乳头状瘤,可显著减轻患者术后疼痛和减少术中出血量及手术时间,且术后复发率较低,而鼻部手术史、肿瘤生发中心和临床分级作为影响术后复发的独立危险因素。

Objective To investigate the clinical effect and recurrence factor analysis of nasal endoscopy.Methods 120 patients with nasal sinus inverted papillary tumor were divided into research group(60 cases)and control group(60 cases)according to randomized digital table.The control group performed traditional nasal anatomy,and the research group performed nasal endoscopic nasal anatomy.The two indexes of surgical time,intraoperative bleeding volume,VAS rate,VAS rate],recurrence rate and the use of logistics regression model.Results Surgical time,intraoperative bleeding volume and postoperative VAS scores were significantly lower than the control group(P<0.05)and no significant difference in surgical success rate(P>0.05);the recurrence rate was significantly lower than the control group(8.34%vs 25.00%)(P<0.05);multifactor logistics regression showed nasal surgery history,tumor germinal center and clinical classification as independent risk factors(P<0.05).Conclusion Nasal endoscopic anatomy for sinus papilloma can significantly reduce postoperative pain,postoperative bleeding volume and surgical time,and the postoperative recurrence rate is low,while nasal surgery history,tumor germinal center and clinical classification should be timely intervention supported as independent risk factors affecting postoperative recurrence.
