经腹超声联合经阴道超声早期鉴别异位妊娠的临床价值分析

Analysis of the Clinical Value of Transabdominal Ultrasound Combined with Transvaginal Ultrasound in the Early Identification of Ectopic Pregnancy

目的:探讨经腹超声联合经阴道超声早期鉴别异位妊娠的临床价值。方法:选取2018年9月-2020年9月收治的70例疑似异位妊娠的患者,所有患者均行经腹超声、经阴道超声检查,评价经腹超声联合经阴道超声早期鉴别异位妊娠的价值。结果:70例患者经手术病理确诊阳性56例,阴性14例,经腹部超声确诊阳性43例,阴性6例,经阴道超声确诊阳性46例,阴性10例,联合超声确诊阳性53例,阴性12例;经腹部超声联合经阴道超声诊断准确率、特异度、敏感度分别为92.86%、85.71%、94.64%,明显较经腹部超声、经阴道超声单项检查高(P<0.05);经腹部超声联合经阴道超声对附件包块、胚芽、宫内假孕囊、盆腔积液检出率分别为78.57%、74.29%、71.43%、80.00%,明显较经腹部超声、经阴道超声单项检查高(P<0.05)。结论:经腹超声联合经阴道超声在异位妊娠早期鉴别中临床价值较高,可提高诊断准确度、特异度及敏感度,值得应用。

Objective:To explore the clinical value of transabdominal ultrasound combined with transvaginal ultrasound in the early identification of ectopic pregnancy.Methods:70 patients with suspected ectopic pregnancy admitted to a hospital from September 2018 to September 2020 were selected.All patients underwent transabdominal ultrasound and transvaginal ultrasound to evaluate the value of transabdominal ultrasound combined with transvaginal ultrasound in the early identification of ectopic pregnancy.Results:Among 70 patients,56 cases were positive and 14 cases were negative according to the surgical pathology,43 cases were confirmed positive and 6 cases were confirmed negative by transabdominal ultrasound,46 cases were confirmed positive and 10 cases were confirmed negative by transvaginal ultrasound,and 53 cases were confirmed positive and 12 cases were confirmed negative by combined ultrasound.The diagnostic accuracy,specificity,and sensitivity of transabdominal ultrasound combined with transvaginal ultrasound were 92.86%,85.71%,and 94.64%,respectively,which were significantly higher than those of transabdominal ultrasound and transvaginal ultrasound alone(P<0.05).The detection rates of transabdominal ultrasound combined with transvaginal ultrasound for adnexal mass,embryo,intrauterine pseudopregnancy sac,and pelvic effusion were 78.57%,74.29%,71.43%,and 80.00%,which were significantly higher than those of transabdominal ultrasound and transvaginal ultrasound alone(P<0.05).Conclusion:Transabdominal ultrasound combined with transvaginal ultrasound has a high clinical value in the early identification of ectopic pregnancy,which can improve the accuracy,specificity and sensitivity of diagnosis and is worthy of application.
