前馈控制结合音乐疗法对腹腔镜胆囊切除术后患者应激反应、康复积极性、应对方式的影响

Effects of feedforward control combined with music therapy on stress response,rehabilitation enthusiasm,coping style in patients after laparoscopic cholecystectomy

目的 探讨前馈控制结合音乐疗法对腹腔镜胆囊切除术后患者应激反应、康复积极性、应对方式的影响。方法选取2020年3月至2021年3月我院收治的100例腹腔镜胆囊切除术后患者为研究对象,根据随机数字表法将其分为对照组和观察组,各50例。对照组采用常规护理干预,观察组采用前馈控制结合音乐疗法进行护理干预。比较两组的干预效果。结果 干预后,观察组的C反应蛋白(CRP)、肾上腺素(E)、皮质醇(Cor)水平低于对照组,差异具有统计学意义(P<0.05)。干预后,观察组的恐惧视觉模拟评分法(FAVS)、视觉模拟评分法(VAS)评分低于对照组,差异具有统计学意义(P<0.05)。干预后,观察组的积极举动、康复护理配合度评分高于对照组,放弃举动评分低于对照组(P<0.05)。观察组的拔管时间、住院时间短于对照组,胆囊功能评分、苏醒体温高于对照组,差异具有统计学意义(P<0.05)。观察组的并发症总发生率低于对照组,差异具有统计学意义(P<0.05)。结论 前馈控制结合音乐疗法有利于减轻腹腔镜胆囊切除术后患者的应激反应,提高康复积极性,缩短麻醉恢复时间,减少术后并发症的发生。

Objective To investigate the effects of feedforward control combined with music therapy on stress response,rehabilitation enthusiasm,coping style in patients after laparoscopic cholecystectomy.Methods One hundred cases of patients after laparoscopic cholecystectomy admitted in our hospital from March 2020 to March 2021 were selected as the research objects,the patients were divided into control group and observation group according to the random number table method,with 50 cases in each group.The control group received routine nursing intervention,and the observation group received feedforward control combined with music therapy for nursing intervention.The intervention effects of the two groups were compared.Results After intervention,the levels of C-reaction protein(CRP),epinephrine(E) and cortisol(Cor) in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After intervention,the scores of Fear Visual Analog Scale(FAVS) and Visual Analogue Scales(VAS)in the observation group were lower than those in the control group,and the differences were statistically significant(P <0.05).After intervention,the scores of positive action and rehabilitation nursing cooperation in the observation group were higher than those in the control group,and the score of abandonment action was lower than that in the control group,and the differences were statistically significant(P<0.05).The extubation time and hospitalization time of the observation group were shorter than those of the control group,and the gallbladder function score and awakening body temperature were higher than those of the control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P <0.05).Conclusion Feedforward control combined with music therapy is conducive to reducing the stress response of patients after laparoscopic cholecystectomy,improving rehabilitation ehthusiasm,shortening the recovery time of anesthesia,and reducing the occurrence of postoperative complications.
