刘健从脾肾论治强直性脊柱炎经验

Experience of LIU Jian in Treating Ankylosing Spondylitis from Spleen and Kidney

强直性脊柱炎属中医“肾痹”“大偻”,多表现为腰背部疼痛、晨僵,甚至累及外周关节,导致关节活动受限、畸形。刘健教授认为强直性脊柱炎属本虚标实之证,肾督空虚、脾气不足为本,湿热互结、脉络瘀阻为标,先后两天亏虚,督脉失养,湿热痰瘀互结,脉络不通,则发为强直。治疗上常投补肾健脾药,合以清热利湿、化瘀通络药,脾旺则痰湿得化,先天得资,瘀去则邪热得清,脉络得通,故以补肾健脾、清热通络为根本治疗原则,收效颇甚。

Ankylosing spondylitis is a Chinese medicine “kidney Bi”,“big hunchback”,mostly manifested as lumbar back pain,morning stiffness,or even involving peripheral joints,resulting in joint activity limited,deformity.LIU Jian believes that ankylosing spondylitis is a syndrome of deficiency and deficiency,which is based on vacuity of kidney and deficiency of temper,and is characterized by damp-heat interaction and blood stasis of veins.Spleen and kidney deficiency,loss of nourishment,damp-heat phlegm and blood stasis each other,vein impropriety,hair is tonic.In the treatment,kidney tonifying and spleen strengthening drugs are often used,combined with heatclearing and dampness,stasis removing and collaterals removing.If the spleen is strong,the phlegm and dampness will be changed,and the innate endowment will be obtained.If the blood stasis is gone,the evil heat will be cleared,and the veins will be connected.Therefore,tonifying the kidney and strengthening the spleen,clearing heat and clearing collaterals are the basic treatment principles,and the results are quite good.
