基于古代噎膈专药探索现代食管癌诊疗

Exploration of Modern Diagnosis and Treatment of Esophageal Cancer based on Ancient Special Herbs for Dysphagia

目的:对中医古籍中治疗噎膈的专药进行挖掘并分析其用药规律,以探索现代食管癌相关中医临床研究。方法:挖掘《中华医典》数据库中治疗噎膈的专药,参考六本现行中药标准书籍对专药的性味归经和功效进行数据整理并进行数据分析。结果:共挖掘出符合入选标准的噎膈专药76种。药性以温、寒、平多见;药味以辛、甘、苦多见;归经以入脾、胃、肺、肝、肾经多见;药物功效以理气、止痛、解毒、补中、清热、温里、化痰多见。结论:食管癌病性可能为寒热胶结,病因可能包括气滞、痰湿、瘀血和热毒,病位可能涉及脾、胃、肺、肝、肾,处方用药可考虑以辛开、苦降、甘和为则。

Objective:To excavate the special medicines for the treatment of choking diaphragm in the ancient books of traditional Chinese medicine and analyze their medication rules,so as to explore the clinical research of modern Chinese medicine related to esophageal cancer.Methods:The special medicines for the treatment of choking diaphragm in the database of “Chinese Medical Dictionary” were excavated,and the data of the properties,taste,meridian and efficacy of special medicines were sorted and analyzed with reference to six current standard books of traditional Chinese medicine.Results:A total of 76 special drugs for choking diaphragm were excavated that met the inclusion criteria.The properties of medicine are mostly warm,cold and flat;the taste of medicine is mostly pungent,sweet and bitter;the meridian is more commonly used to enter the spleen,stomach,lung,liver and kidney meridians;the efficacy of medicine is to regulate Qi,relieve pain,detoxify,invigorate the middle and clear away heat,Wen Li,and phlegm are more common.Conclusion:The pathogenesis of esophageal cancer may be cold and heat cementation.The causes may include stagnation of Qi,phlegm-dampness,blood stasis and heat toxin.The disease location may involve the spleen,stomach,lung,liver and kidney.Prescription drugs can be considered with Xin Kai and Ku Jiang and Gan He is the rule.
